“伏邪”理论指导下祛风逐瘀化痰方治疗偏头痛疗效及对颅内血流状态及血管舒缩因子的影响

摘　　要：目的:分析偏头痛患者应用“伏邪”理论指导下的祛风逐瘀化痰方治疗的效果及其对颅内血流状态及血管舒缩因子的影响。方法:选取符合标准的102例偏头痛患者作为研究对象,以随机数字表法分组。基础组51例给予盐酸氟桂利嗪胶囊治疗,实验组51例增加“伏邪”理论指导下的祛风逐瘀化痰方治疗。治疗4周后,比较2组患者偏头疼发作情况、颅内血流、血管舒缩因子、不良反应。结果:实验组患者发作次数、周平均发作时间、疼痛程度均低于基础组(P<0.05);实验组患者椎动脉、基底动脉、大脑前动脉、大脑中动脉平均血流速度均低于基础组(P<0.05);实验组患者核转录因子κB、内皮素1、降钙素基因相关肽、一氧化氮水平均低于基础组(P<0.05);2组治疗过程中均未出现明显不良反应。结论:“伏邪”理论指导下的祛风逐瘀化痰方能够有效改善偏头痛患者偏头疼发作情况,降低颅内血流速度,调节血管舒缩因子水平,且安全性高。Objective:To analyze the effect of Qufeng Zhuyu Huatan prescription under the guidance of“Fu Xie”theory on migraine patients and its effect on intracranial blood flow and vasomotor factors.Methods:A total of 102 patients with migraine who met the criteria were selected as the research subjects and divided into groups by random number table method.The basic group(51 cases)was treated with Flunarizine Hydrochloride Capsules,and the experimental group(51 cases)was treated with Qufeng Zhuyu Huatan prescription under the guidance of“Fu Xie”theory.After 4 weeks of treatment,the migraine attacks,intracranial blood flow,vasomotor factors and adverse reactions of the two groups were compared.Results:The number of attacks,average weekly attack time and pain severity of the experimental group were lower than those of the basic group(P<0.05);the average blood flow velocity of the vertebral artery(VA),basilar artery(BA),anterior cerebral artery(ACA)and middle cerebral artery(MCA)of the experimental group was lower than that of the basic group(P<0.05);the levels of nuclear transcription factor-κB(NF-κB),Endothelin 1(ET-1),Calcitonin gene-related peptide(CGRP)and nitric oxide(NO)of the experimental group were lower than those of the basic group(P<0.05);no obvious adverse reactions occurred in the two groups during the treatment.Conclusion:Qufeng Zhuyu Huatan prescription guided by the theory of“Fu Xie”can effectively improve the migraine attacks of migraine patients,reduce intracranial blood flow velocity,regulate the level of vasomotor factors,and has high safety.
