基于“久痛入络”理论探究血府逐瘀汤联合射频脉冲治疗老年带状疱疹后遗神经痛

摘　　要：目的:基于“久痛入络”理论探究血府逐瘀汤联合射频脉冲治疗老年带状疱疹后遗神经痛PHN。方法:将72例PHN患者随机分组为观察组和对照组,每组36例,对照组进行射频脉冲手术,观察组使用射频脉冲加以血府逐瘀汤。观察患者在手术前,手术后第2天,手术后1个月、2个月、3个月疼痛评分变化,并比较两组患者治疗前后色沉的面积颜色、疼痛和麻木相关症状的改善情况及临床有效率、炎症、凝血指标、红外热像差值的变化情况。结果:疗程结束后,两组术后1、2及3月,疼痛评分(VAS)较前一次均降低,且观察组优于对照组(P<0.05);观察组在改善色沉面积、颜色,麻木情况中较优于对照组(P<0.05);观察组在炎症因子,凝血等相关指标改善表现中较优于对照组(P<0.05);观察组健患侧红外热像差值较对照组差值明显降低(P<0.05)。结论:血府逐瘀汤联合射频脉冲治疗在改善机体炎症水平、病理状态及疼痛感觉异常方面有良好的临床疗效。Objective:For exploration the clinical effectiveness of combining Xuefu Zhuyu Tang pf radio frequency pulse in healing PHN.Methods:Seventy-two PHN patients were equally allocated two group,one is observation group,and the other was control group.The control group underwent radio frequency pulse exclusively,whereas the observation group received oral administration of Xuefu Zhuyu Tang alongside the surgery.Pain scores were recorded before surgery,on the second day post-surgery,and at 1、2 and 3 months post-surgery.The observation group showed better improvement in color sinking area,color,and numbness compared to the control group,as well as changes in clinical efficacy,inflammation levels with coagulation indicators.Results:after the treatment period,the observation group express a superior clincal efficacy rate than the others(P<0.05)and the VAS pain grade at1、2 and 3 months were reduced in two groups.when compared to baseline,the observation group displayed better transmutation than the others(P<0.05).with more marked amelioration in skin discoloration,color restoration,and numbness alleviation(P<0.05).the observation group express an benefit than the control group in aspect of the levels of inflammatory factors coagulation indicators(P<0.05).Conclusion:The Xuefu Zhuyu Tang along with radiofrequency pulse therapy has shown good clinical efficacy in improving the level of inflammation,pathological status,and abnormal pain sensation in the body.
