摘  要:目的探讨在体外受精(in vitro fertilization,IVF)胚胎培养过程中时差成像(Time-lapse)培养与常规培养方式对胚胎发育及妊娠结局的影响。方法回顾性分析2022年7月—2023年7月接受体外受精/卵泡浆内单精子注射-胚胎移植(in vitro fertilization/intracytoplas-mic sperm injection-embryo transfer,IVF/ICSI-ET)治疗的118对夫妇(118个IVF/ICSI-ET周期)的临床资料;按照胚胎不同培养方法分为对照组(常规培养,n=66)和研究组(Time-lapse培养,n=52),比较两组胚胎质量、受试者孕初期激素水平[人绒毛膜促性腺激素(human chorionic gonadotropin,HCG)、雌二醇(estradiol,E_(2))、孕酮(progesterone,P)]、妊娠结局及新生儿情况。结果两组的一般资料比较,差异无统计学意义(P>0.05);研究组受试者孕初期的HCG、E_(2)、P水平均高于对照组(P<0.05);两组获卵数、正常受精数、可移植胚胎数等胚胎质量比较差异无统计学意义(P>0.05),研究组优质胚胎数、囊胚形成数等胚胎质量优于对照组(P<0.05);研究组妊娠率高于对照组(P<0.05),研究组活产率高于对照组(P<0.05),两组流产率比较差异无统计学意义(P>0.05);研究组与对照组早产率、不良新生儿结局、畸形、巨大儿、低体重儿等新生儿情况比较差异无统计学意义(P>0.05)。结论Time-lapse培养技术可提高IVF/ICSI-ET的优质胚胎数、囊胚形成数、妊娠率和活产率,且不增加新生儿不良结局的风险。Objective To explore the effects of Time-lapse imaging(TLI)culture and routine culture on embryo development and pregnancy outcomes during in vitro fertilization(IVF)-embryo culture.Methods A retrospective analysis was performed on the clinical data of 118 couples who underwent IVF/intracytoplasmic sperm injection-embryo transfer(IVF/ICSI-ET)from July 2022 to July 2023.According to different embryo culture methods,the couples were divided into control group(routine culture,n=66)and study group(Time-lapse culture,n=52).Two groups were compared for embryo quality,the levels of hormones[human chorionic gonadotropin(HCG),estradiol(E_(2)),and progesterone(P)]of the subjects in the first trimester,pregnancy outcomes and neonatal conditions.Results There was no statistically significant difference in general data between two groups(P>0.05).In the first trimester,the levels of HCG,E_(2),and P were higher in study group than those in control group(P<0.05).There was no statistically significant difference in embryo quality(the number of retrieved eggs,normal fertilization and transferable embryos)between two groups(P>0.05).The numbers of high-quality embryos and blastocyst formation were better in study group than those in control group(P<0.05).The pregnancy rate of study group was higher than that of control group(P<0.05),and the live birth rate of study group was higher than that of control group(P<0.05).There was no statistically significant difference in the miscarriage rate between two groups(P>0.05).There were no statistically significant differences in the conditions of newborns(premature birth rate,adverse neonatal outcomes,deformities,macrosomia and low-birth-weight infants between study and control groups(P>0.05)).Conclusion Time-lapse culture can increase the numbers of high-quality embryos and blastulation,pregnancy rate,and live birth rate in IVF/ICSI-ET,with low risk of neonatal adverse outcomes.
