子午流注督灸联合泗滨砭石耳部刮痧对腰椎间盘突出症患者ODI评分及腰椎活动度的影响

摘　　要：目的探讨子午流注督灸联合泗滨砭石耳部刮痧治疗腰椎间盘突出症(LDH)患者的临床疗效,并分析其对腰椎功能、腰椎活动度的影响。方法回顾性分析2022-01—2024-01本院收治的150例LDH患者的临床资料,依据治疗方案分为3组,各50例。对照1组予以泗滨砭石耳部刮痧治疗,对照2组予以子午流注督灸治疗,研究组予以子午流注督灸联合泗滨砭石耳部刮痧治疗,连续治疗4周。统计对比3组临床疗效及治疗前后中医证候积分、腰椎功能[Oswestry功能障碍指数问卷(ODI)、日本骨科学会(JOA)评分]、疼痛程度[视觉模拟评分法(VAS)]、腰椎活动度、炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、前列腺素E2(PGE2)]水平、氧化应激指标[超氧化物歧化酶(SOD)、丙二醛(MDA)、谷胱甘肽过氧化物酶(GSH-Px)]。对比3组不良反应。结果研究组治疗后总有效率、JOA、血清SOD和GSH-Px水平高于对照1组、对照2组,中医证候积分、ODI、VAS评分和血清TNF-α、IL-1β、IL-6、PGE2、MDA等水平均低于对照1组、对照2组,腰椎活动度改善程度优于对照1组、对照2组,差异均有统计学意义(P<0.05);3组治疗期间均无明显不良反应发生。结论子午流注督灸联合泗滨砭石耳部刮痧治疗LDH的疗效显著,可改善临床症状、腰椎活动度,促进腰椎功能恢复,减轻疼痛程度,抑制炎症-氧化应激反应,且具有一定安全性。Objective To investigate the clinical efficacy of midnight-noon ebb-flow Du moxibustion combined with Sibin stone therapy ear scraping in the treatment of patients with lumbar disc herniation(LDH),and to analyze its impact on lumbar function and lumbar mobility.Methods A total of 150 cases of LDH patients in our hospital from January 2022 to January 2024 were selected and retrospectively analyzed.They were divided into control group 1,control group 2 and study group according to the treatment schemes,with 50 cases in each group.The control group 1 was treated with Sibin stone therapy ear scraping,the control group 2 was treated with midnight-noon ebb-flow Du moxibustion,and the study group was treated with midnight-noon ebb-flow Du moxibustion combined with Sibin stone therapy ear scraping,continuous treatment for 4 weeks.The clinical efficacy of the three groups was compared,as well as TCM syndrome score,lumbar function[Oswestry disability index(ODI),Japanese Orthopedic Association(JOA)],pain degree[visual analogue scale(VAS)],lumbar activity,inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6),prostaglandin E2(PGE2)],oxidative stress indexes[superoxide dismutase(SOD),malondialdehyde(MDA),glutathione peroxidase(GSH-Px)]before and after treatment.The adverse reactions of the three groups were compared.Results The total effective rate、JOA、serum SOD and GSH-Px of the study group after treatment was higher than that of the control group 1 and control group 2;the TCM syndrome score、ODI and VAS、serum TNF-α、IL-1β、IL-6 and PGE2、MDA of the study group was lower than that of the control group 1 and control group 2,the improvement of lumbar activity in the study group was better than that in the control group 1 and control group 2,The differences were all statistically significant(P<0.05);there was no significant difference in adverse reactions occurred in the three groups during treatment.Conclusion Midnight-noon ebb-flow Du moxibustion combined with Sibin stone
