腰硬联合麻醉与连硬外麻醉在阴式子宫全切术中的应用效果比较

摘　　要：目的:分析连硬外麻醉(EA)与腰硬联合麻醉(CSEA)在子宫全切术中的麻醉状况。方法:研究时间段为2022年2月至2023年9月,研究对象选择在本院进行子宫全切术治疗的61例患者,将所有患者依据麻醉方式分为两组,研究组予以腰硬联合麻醉,包含30例患者,对照组采用连硬外麻醉,有31例患者,对两组患者的麻醉期血气指标水平、苏醒时间和不良反应发生状况进行评估和对比。结果:研究组麻醉后5min血气指标高于对照组,拔管5min后血气指标水平研究组也较对照组高,但对比差异没有意义,P>0.05,研究组自主呼吸恢复时间、撤管时间和苏醒时间均小于对照组,研究组不良反应发生率低于对照组,比较差异均有统计学意义(P<0.05)。结论:子宫全切术采用腰硬联合麻醉效果优于连硬外麻醉,血气指标水平没有明显差异,但苏醒时间和不良反应发生率腰硬联合麻醉明显较小,可见其在临床中的麻醉应用价值。Objective:To analyze the anesthesia status of combined epidural anesthesia(EA)and combined spinal-epidural anesthesia(CSEA)in total hysterectomy.Methods:The study period was from February 2022 to September 2022.61 patients who underwent total hysterectomy at Zhangdian District Hospital of traditional Chinese medicine in Zibo.All patients were divided into two groups according to the type of anesthesia,the study group was given combined spinal-epidural anesthesia,including 30 patients,while the control group was given combined epidural anesthesia,including 31 patients,to evaluate and compare the blood gas indexes,the time of recovery and the adverse reaction of the two groups.Results:The Blood Gas Index of the exploration group was higher than that of the control group 5 minutes after anesthesia,and the blood gas index level of the exploration group was also higher than that of the control group 5 minutes after extubation,but there was no significant difference between the two groups(P>0.05),the recovery time of spontaneous respiration,the time of tube withdrawal and the time of recovery in the study group were shorter than those in the control group,and the incidence of adverse reactions in the study group was lower than that in the control group,with significant difference(P<0.05).Conclusion:The effect of combined spinal-epidural anesthesia for total hysterectomy is better than that of combined spinal-epidural anesthesia.There is no significant difference in blood gas,but the time of recovery and the incidence of adverse reactions are significantly lower,therefore,it can be used widely in clinical anesthesia.
