血清CA125、HE4、CEA在上皮性卵巢癌患者中的表达及对预后的预测价值分析

摘　　要：目的:探讨血清糖链抗原125(CA125)、人附睾蛋白4(HE4)、癌胚抗原(CEA)在上皮性卵巢癌患者中的表达,分析其对预后的预测价值。方法:选取2020年3月至2024年3月本院收治的上皮卵巢肿瘤患者130例为研究组(根据病理情况分为良性病变组60例、上皮性卵巢癌组70例),收集同期健康体检人群70例为对照组。比较三组研究对象血清CA125、HE4、CEA水平;对上皮性卵巢癌患者均行腹腔镜手术治疗,根据术后1年是否出现复发或转移分为预后良好(n=47)和不良预后(n=23)两组,多因素Logistic回归分析上皮性卵巢癌患者不良预后的影响因素,采用受试者工作特征(ROC)曲线分析血清CA125、HE4、CEA水平对上皮性卵巢癌患者不良预后的预测价值。结果:上皮性卵巢癌组血清CA125、HE4、CEA水平高于良性病变组与对照组,良性病变组各指标水平高于对照组(P<0.05);预后良好组与不良预后组的国际妇产科联盟(FIGO)分期、血清CA125、HE4、CEA水平之间的差异有统计学意义(P<0.05);Logistic回归分析显示:FIGO分期、血清CA125、HE4、CEA水平是上皮性卵巢癌患者不良预后的影响因素;ROC曲线显示,三项联合预测的灵敏度为96.44%,曲线下面积(AUC)为0.922,联合预测价值最高。结论:血清CA125、HE4、CEA在上皮性卵巢癌中可呈高表达,且为影响患者预后的影响因素,同时三项血清联合检测对上皮性卵巢癌患者术后预后具有较高预测价值。Objective:To investigate the expression of serum glycochain antigen 125(CA125),human epididymal protein 4(HE4)and carcinoembryonic antigen(CEA)in epithelial ovarian cancer patients,and to analyze their prognostic value.Methods:130 patients with epithelial ovarian tumor admitted to our hospital from March 2020 to March 2024 were selected as the study group divided into benign lesion group(60 cases)and epithelial ovarian cancer group(70 cases)according to pathological conditions,and 70 healthy people were collected as the control group during the same period.The serum levels of CA125,HE4 and CEA were compared among the three groups.All patients with epithelial ovarian cancer were treated by laparoscopic surgery,and were divided into two groups with good prognosis(n=47)and poor prognosis(n=23)according to whether recurrence or metastasis occurred one year after surgery.Multivariate Logistic regression analysis was performed to analyze the influencing factors of poor prognosis in patients with epithelial ovarian cancer.The predictive value of serum CA125,HE4 and CEA levels in patients with epithelial ovarian cancer was analyzed by receiver operating characteristic(ROC)curve.Results:The serum levels of CA125,HE4 and CEA in epithelial ovarian cancer group were higher than those in benign lesion group and control group,and the indexes in benign lesion group were higher than those in control group(P<0.05).FIGO stage,serum CA125,HE4 and CEA levels were significantly different between the good prognosis group and the poor prognosis group(P<0.05).Logistic regression analysis showed that FIGO stage,serum CA125,HE4 and CEA levels were the factors affecting the poor prognosis of epithelial ovarian cancer patients.The ROC curve showed that the sensitivity of the three combined predictions was 96.44%,the area under the curve(AUC)was 0.922,and the combined prediction value was the highest.Conclusion:Serum CA125,HE4 and CEA can be highly expressed in epithelial ovarian cancer,and they are the influential factors affecting the prog
