摘　　要：目的:分析超声引导下PICC导管治疗儿童急性化脓性膝关节炎的临床疗效。方法:回顾性分析长春市儿童医院2016年5月至2022年12月收治的20例儿童急性化脓性膝关节炎的临床资料,其中男性14例,女性6例,年龄1~12岁,均为单侧发病,左侧12例,右侧8例。入院后择期在全麻下采用PICC导管在超声引导下穿刺,持续灌洗引流7~9天后拔管。记录患儿生命体征、疼痛视觉模拟评分、Lysholm膝关节评分、膝关节活动度以及相关感染指标。结果:20例患儿手术顺利,术中未出现并发症及其他组织结构的损伤,术后恢复良好。所有患儿均获得随访,随访6~36个月,2例患儿出现膝关节屈曲轻度受限,余未见膝关节活动障碍或下肢短缩无力等并发症。结论:采用超声引导下PICC导管治疗儿童急性化脓性关节炎短期临床疗效显著,且具有安全、简便、损伤小,恢复快及远期收益大的优点。Objective:To analyze the clinical efficacy of ultrasound-guided PICC catheter in the treatment of acute septic knee osteoarthritis in children.Methods:The clinical data of 20 cases of acute septic knee osteoarthritis in children admitted to Changchun Children's Hospital from May 2016 to December 2022 were retrospectively analyzed,among which there were 14 cases of males and 6 cases of females,with the age ranging from 1 to 12 years old[3(2.0,9.1)years old],and all of them had unilateral onset of disease,with 12 cases on the left side and 8 on the right side.The PICC catheter was punctured under general anesthesia with ultrasound guidance at an elective stage after admission,and the catheter was removed after 7~9 days of continuous irrigation and drainage.The children's vital signs,pain visual analog scores,Lysholm knee scores,knee mobility and related infection indicators were recorded.Results:Twenty children underwent successful surgery without intraoperative complications or other tissue damage,and had good postoperative recovery.All the children were followed up without complications such as recurrence,knee mobility disorder or lower limb shortening weakness.Conclusion:The use of ultrasound-guided PICC catheter for the treatment of acute septic arthritis in children has remarkable short-term clinical efficacy,and is safe,simple,with little injury,fast recovery,low cost and large long-term benefits for the children.
