老十针治疗功能性消化不良肝胃不和证临床观察

摘　　要：目的 探讨“老十针”治疗肝胃不和型功能性消化不良(FD)的临床效果。方法 选取2022年3月—2023年2月在阳江市人民医院中医理疗门诊就诊的肝胃不和型FD患者90例,采用随机数字表法分为西药组、针灸组、西药+针灸组,各30例。西药组给予枸橼酸莫沙必利治疗,针灸组采用老十针穴位处方治疗,西药+针灸组在西药基础上联合针灸治疗,各组均连续治疗2周,观察三组患者临床疗效,治疗前后的中医证候积分及血清学指标水平。结果 西药+针灸组治疗总有效率93.33%(28/30)明显优于西药组的66.67%(20/30)和针灸组的73.33%(22/30)(P<0.05)。治疗后,三组患者痞满、胸胁胀痛、嗳气、反酸等中医证候总积分明显降低(P<0.05);治疗后,三组患者血清学指标均有所改善,胃动素(MTL)、生长抑素(SS)水平均有显著升高,胃泌素(GAS)、5-羟色胺(5-HT)水平明显下降,且西药+针灸组改善情况优于其余两组(P<0.05)。结论 老十针联合西药治疗肝胃不和型FD效果显著,能明显缓解患者临床症状,改善GAS、SS、MTL、5-HT等血清学指标水平,值得临床推广。Objective To explore the clinical efficacy of experienced ten acupoints in the treatment of syndrome of incoordination be-tween liver and stomach of functional dyspepsia.Methods From March 2022 to February 2023,90 patients with syndrome of incoordi-nation between liver and stomach of functional dyspepsia who attended the TCM physiotherapy clinic of Yangjiang People's Hospital were selected.They were randomly divided into Western medicine group,acupuncture group,and Western medicine combined with acupuncture group,with 30 cases in each group.The Western medicine group was given mosapride treatment,the acupuncture group was prescribed treatment with experienced ten acupoints acupuncture points,and the Western medicine combined with acupuncture group received acupuncture treatment on the basis of Western medicine.Afer 2 weeks of continuous treatment,the clinical symptoms,TCM syndrome points,GAS,SS and other serological indexes of the three groups before and after treatment were observed.Results The total effective rate of 93.33%(28/30)in the Westem medicine combined with acupuncture group was significantly higher than that of 66.67%(20/30)in the Western medicine group and 73.33%(22/30)in the acupuncture group(P<0.05).After treatment,the scores of TCM syndrome such as fullness,chest and hypochondriac distending pain,belching and acid reflux were significantly decreased in the three groups(P<0.05).After treatment,the serological indexes of the three groups were improved,the levels of motilin(MTL)and so-matostatin(SS)were significantly increased,the levels of gastrin(GAS)and 5-hydroxytryptamine(5-HT)were significantly decreased,and the improvement of the Western medicine combined with acupuncture group was better than that of the other two groups(P<0.05).Conclusion Experienced ten acupoints combined with Western medicine has a significant effect in the treatment of patients with syn-drome of incoordination between liver and stomach of functional dyspepsia,can significantly alleviate the clinical symptoms of patients an
