外周性骨关节布鲁氏菌病的临床特点及MRI分析

摘　　要：目的探讨外周性骨关节布鲁氏菌病(osteoarticular brucellosis,OAB)磁共振成像(magnetic resonance imaging,MRI)表现,以提高对该病的认识。方法回顾性分析2019年8月至2021年8月41例经临床确诊OAB患者的临床资料及MRI表现,评价OAB累及关节、骨及软骨异常、关节周围滑膜炎、肌腱/腱鞘炎、软组织肿胀情况与临床分期的相关性。结果41例共58个外周关节,包括24个膝关节、20个髋关节、8个踝关节、5个肩关节、1个肘关节。MRI表现增生硬化/骨髓水肿27个,关节软骨损伤/关节间隙狭窄12个,滑膜囊积液44个,腱鞘积液10个,周围软组织肿胀35个。不同临床分期间骨质改变的发生率差异有统计意义(P<0.05),关节软骨、滑膜、肌腱/腱鞘及周围软组织病变在不同病程发生率差异无统计学意义(P>0.05)。结论MRI对诊断外周性OAB及评估病变侵犯程度具有重要应用价值。Objective To investigate the clinical and magnetic resonance imaging(MRI)manifestations of peripheral osteoarthrosis brucellosis(OAB)in order to improve the understanding of OAB.Methods Clinical data and MRI images of 41 clinically diagnosed OAB patients from August 2019 to August 2021,including involved joints,bone and cartilage abnormalities,periarticular synovitis,myotenositis/tenosynovitis and soft tissue swelling,with their correlations with clinical stages,were retrospectively analyzed.Results 58 peripheral joints in 41 patients were scanned,including 24 knee joints,20 hip joints,8 ankle joints,5 shoulder joints and 1 elbow joint.MRI showed hyperplastic sclerosis/bone marrow edema in 27 cases,articular cartilage injury/joint space stenosis in 12 cases,synovial sac effusion in 44 cases,tenosynovial effusion in 10 cases,and surrounding soft tissue swelling in 35 cases.There were statistically significant differences in the incidence of bone abnormalities in different clinical periods(P<0.05),while there were no statistically significant differences in the incidence of articular cartilage,synovium,tendon/tendon sheath and surrounding soft tissue abnormalities in different clinical stages(P>0.05).Conclusion MRI has important application value in the diagnosis of peripheral OAB and the evaluation of lesion invasion degree.
