慢性阻塞性肺疾病筛查问卷在宁夏基层医疗卫生机构中应用效果评价

摘　　要：目的:评价慢性阻塞性肺疾病(以下简称慢阻肺)筛查问卷在宁夏回族自治区基层医疗卫生机构中的应用价值。方法:对2023年8月1日—12月31日在固原市人民医院、吴忠市利通区胜利社区卫生服务中心就诊的631名≥35岁居民开展问卷筛查、身体测量和临床检查,判定慢阻肺患者和非慢阻肺患者,应用ROC曲线评价问卷筛查效果。结果:检出慢阻肺患者154例,检出率为24.4%。慢阻肺筛查问卷对慢阻肺和非慢阻肺人群具有较好的区分度(P<0.01)。问卷总分ROC曲线下面积为0.825(95%CI:0.785~0.865,P<0.01)。问卷评分阳性筛查慢阻肺的敏感性、特异性、正确指数、阳性预测值、阴性预测值、阳性似然比和阴性似然比分别为0.779、0.744、0.523、0.496、0.913、3.042和0.297。结论:慢阻肺筛查问卷具有较好的诊断辅助价值,值得在基层医疗卫生机构应用评价。OBJECTIVE To evaluate the application value of COPD population screening questionnaire(COPD-PS)in Ningxia primary health care institutions.METHODS From August 1st to December 31st,2023,631 residents aged 35 and above who went to Shengli Community Health Service Center,Litong District,Wuzhong and Guyuan People’s Hospital received medical treatment,through questionnaire screening,body measurement and clinical examination,to identify COPD patients and non-patients,and use ROC curve to evaluate the screening effect of questionnaire.RESULTS Among the 631 effective samples included in the analysis,154 cases of COPD were detected,the detection rate was 24.4%.COPD-PS questionnaire had a good distinction between COPD and non-COPD population(P<0.01).The total score area under ROC curve was 0.825(95%CI:0.7855~0.865,P<0.01).The sensitivity,specificity,correctness index,positive predictive value,negative predictive value positive likelihood ratio and negative likelihood ratio of positive and negative predictive value were 0.779,0.744,0.523,0.496,0.913,3.042 and 0.297,respectively.CONCLUSION The COPD-PS questionnaire has good diagnostic auxiliary value,and it is worth to be applied and promoted in primary health care institutions.
