摘 要:目的探讨超声新参数静脉管腔横截周长的平方(C^(2))与面积(A)的比值(C^(2)/A)对骨科住院患者下肢深静脉血栓(DVT)的预测价值。资料与方法前瞻性选取2020年6月—2021年6月空军军医大学西京医院骨科无DVT的150例住院患者,术前采用超声检查股总静脉(CFV)、股浅静脉(SFV)和腘静脉(POV)的A、C、C^(2)/A、内径值、静脉流速及血流量,术后超声监测血栓形成情况。对比血栓组和非血栓组的临床资料和超声参数,并绘制受试者工作特征(ROC)曲线评估各参数预测DVT的效能及截断值。结果两组CFV内径、CFV血流量、CFV-C、CFV-A、CFV-C^(2)/A、SFV血流量、SFV-C、SFV-C^(2)/A、POV血流量、POV-C、POV-A、POV-C^(2)/A差异有统计学意义(t=2.64~10.41,P均<0.05)。当超声参数CFV-C^(2)/A、SFV-C^(2)/A、POV-C^(2)/A的截断值分别为>16.01、>16.53、>16.54时,预测DVT的曲线下面积最大,分别为0.906、0.920、0.870,敏感度分别为83.8%、78.4%、81.1%,特异度分别为82.3%、89.4%、85.8%。结论超声新参数C^(2)/A具有较高的DVT预测效能,可为临床开展早期DVT预防提供新的参考信息。Purpose To investigate the predictive value of the ratio of the square of the cross-sectional perimeter(C^(2))to the area(A)of the venous lumen(C^(2)/A)in relation to lower extremity deep venous thrombosis(DVT)among orthopaedic inpatients.Materials and Methods A total of 150 inpatients without DVT from the Orthopedics Department of Xijing Hospital Affiliated to the Air Force Military Medical University were prospectively chosen from June 2020 to June 2021.Before the operation,the parameters including A,C,C^(2)/A,inner diameter values,venous flow velocity and blood flow volume of the common femoral vein(CFV),superficial femoral vein(SFV)and popliteal vein(POV)were examined by ultrasound.After the operation,the formation of thrombus was monitored using ultrasound.Subsequently,the clinical data and ultrasound parameters of the thrombus group and the non-thrombus group were compared.Moreover,the receiver operating characteristic(ROC)curve was plotted to assess the efficacy and cut-off values of each parameter in predicting DVT.Results There were statistically significant differences between the two groups in CFV inner diameter,CFV blood flow,CFV-C,CFV-A,CFV-C^(2)/A,SFV blood flow,SFV-C,SFV-C^(2)/A,POV blood flow,POV-C,POV-A and POV-C^(2)/A(t=2.64-10.41,all P<0.05).When the cut-off values of the ultrasonic parameters CFV-C^(2)/A,SFV-C^(2)/A,and POV-C^(2)/A were>16.01,>16.53,and>16.54 respectively,the area under the curve for predicting DVT was the largest,which were 0.906,0.920,and 0.870 respectively.The sensitivities were 83.8%,78.4%,and 81.1%respectively,and the specificities were 82.3%,89.4%,and 85.8%respectively.Conclusion The novel ultrasonic parameter C^(2)/A exhibits a relatively high predictive efficacy for DVT,and it is capable of furnishing novel reference information for the implementation of early DVT prevention in the clinical setting.
