半夏附子粳米汤加味配合脐火疗法治疗溃疡性结肠炎脾肾阳虚证的临床研究

摘　　要：目的:观察半夏附子粳米汤加味配合脐火疗法治疗溃疡性结肠炎(UC)脾肾阳虚证的临床疗效。方法:选取2023年1月至2024年1月,本院收治的90例脾肾阳虚型UC患者,采用随机数字表法分为治疗组和对照组,每组各45例。对照组给予美沙拉嗪肠溶片口服治疗,治疗组在对照组的基础上联合半夏附子粳米汤加味配合脐火疗法治疗。连续治疗28 d,比较2组临床疗效,治疗前后中医证候评分等。结果:治疗组总有效率为93.33%,高于对照组的84.44%(P<0.05)。治疗前,2组中医证候评分比较差异无统计学意义(P>0.05),治疗后,2组中医证候评分均明显降低,且治疗组腹痛、腹泻、脓血便、食少纳差、形寒肢冷和腰酸膝软的证候评分均显著低于对照组(P<0.05),2组腹胀评分比较差异无统计学意义(P>0.05)。结论:半夏附子粳米汤加味配合脐火疗法治疗脾肾阳虚型UC临床疗效肯定,可有效改善患者中医证候,值得临床推广应用。Objective To observe the clinical efficacy of supplemented Banxia Fuzi Jingmi Decoction combined with umbilical fire therapy in the treatment of ulcerative colitis(UC)with syndrome of spleen-kidney yang deficiency.Methods Total 90 UC patients with syndrome of spleen-kidney yang deficiency admitted to our hospital from January 2023 to January 2024 were selected and divided into the treatment group and the control group by random number table method,with 45 cases in each group.The control group was given oral treatment with mesalazine enteric-coated tablets,and the treatment group was treated with supplemented Banxia Fuzi Jingmi Decoction combined with umbilical fire therapy on the basis of the control group.The treatment lasted for 28 days.The clinical efficacy and traditional Chinese medicine(TCM)syndrome scores before and after treatment were compared between the two groups.Results The total effective rate of the treatment group was 93.33%,which was higher than 84.44%of the control group(P<0.05).Before treatment,there was no significant difference in TCM syndrome scores between the two groups(P>0.05).After treatment,the TCM syndrome scores of both groups were significantly reduced,and the syndrome scores of abdominal pain,diarrhea,stool with blood and pus,poor appetite,cold sensation and cold limbs,and soreness and weakness of the waist and knees in the treatment group were significantly lower than those in the control group(P<0.05),and there was no significant difference in the score of abdominal distension between the two groups(P>0.05).Conclusion Supplemented Banxia Fuzi Jingmi Decoction combined with umbilical fire therapy has definite clinical effect in the treatment of UC with syndrome of spleen-kidney yang deficiency,which can effectively improve the TCM syndromes of patients and is worthy of clinical promotion and application.
