摘　　要：目的:全膝关节置换术(total knee arthroplasty,TKA)可以改善关节畸形、减轻疼痛及活动受限,是治疗膝关节晚期严重疾病的主要手段。术后康复护理和功能锻炼是促进关节功能恢复的重要环节。“互联网+护理服务”已成为慢性病康复与延伸护理的一种有效方法。本研究依托互联网医院平台,旨在探讨“互联网+护理服务”模式在TKA术后居家患者中的应用效果。方法:选取2022年3月至2023年12月在江苏省苏北人民医院行TKA的76例患者为研究对象,其中在江苏省互联网护理小程序下单的居家护理患者为试验组,同期出院患者为对照组,每组38例。对照组给予常规出院指导和电话随访,试验组实施“互联网+护理服务”模式。收集患者的临床资料,对2组患者出院时、出院21 d、出院3个月的膝关节功能评分、膝关节主动活动度(active range of motion,AROM)、疼痛评分进行重复测量方差分析。对2组患者满意度评价以及半年内并发症发生情况进行组间比较。结果:试验组膝关节功能评分、AROM和患者满意度均高于对照组(均P<0.05),疼痛评分和并发症发生率均低于对照组(均P<0.05)。结论:“互联网+护理服务”模式的应用可以改善TKA后患者膝关节功能,减少并发症发生,提高患者满意度。该模式可以满足TKA后居家患者日益增长的护理需求,也有利于医疗资源合理分配。未来可以继续完善相关流程和规章制度,扩大服务范围,为更多患者提供便捷、专业的居家护理服务。Objective:Total knee arthroplasty(TKA)is an effective treatment for advanced and severe knee joint diseases,alleviating deformity,pain,and movement limitations.Postoperative rehabilitation and functional exercise are critical for restoring joint function.Internet+nursing services model has proven to be an effective approach for chronic disease rehabilitation and extended care.Relying on the platform of an internet hospital,this study aims to explore the application of the internet+nursing services model for home patients following TKA.Methods:A total of 76 patients who underwent TKA at Jiangsu Subei People’s Hospital between March 2022 and December 2023 were enrolled.Patients receiving home care through the Jiangsu internet nursing services mini-program were assigned to a experimental group(n=38),while contemporaneous discharged patients served as a control group(n=38).The control group received routine discharge guidance and telephone follow-up,while the experimental group received care under the internet+nursing services model.Clinical data were collected,and repeated measures analysis of variance was performed to compare knee joint function scores,active range of motion(AROM),and pain scores at discharge.Patient satisfaction and complication rates within 6 months were compared between groups.Results:Experimental group demonstrated significantly higher knee joint function scores,AROM,and satisfaction levels than the control group(all P<0.05).Pain scores and complication rates were significantly lower in the experimental group compared to the control group(both P<0.05).Conclusion:Application of the internet+nursing services model improved knee joint function,reduced complications,and improved patient satisfaction following TKA.This model meets the growing nursing needs of home patients after TKA and facilitates the rational allocation of medical resources.Future efforts should focus on refining processes and regulations and expanding service coverage to provide convenient and professional home care for more pa
