摘 要:目的:探讨双源CT双能扫描对急性脑梗死介入溶栓取栓术后脑出血及碘造影剂外渗的鉴别诊断价值。方法:选择本院2023年5月至2024年8月期间收治的50例急性脑梗死并行介入溶栓取栓术病人为研究对象,病人均于上述治疗后2 h内接受双源CT双能扫描,24~72 h再进行常规颅脑CT检查,统计分析双源CT双能扫描对单纯脑出血、单纯碘造影剂外渗以及脑出血合并碘造影剂外渗三种不同情况的诊断价值(灵敏度、特异度、准确度)。结果:常规颅脑CT在治疗后24~72 h内诊断单纯脑出血3例、单纯碘造影剂外渗25例、脑出血合并碘造影剂外渗18例、无脑出血且无造影剂外渗4例。而双源CT术后2 h内双能扫描诊断单纯脑出血和无脑出血且无造影剂外渗全部准确;诊断单纯碘造影剂外渗灵敏度92.00%、特异度96.30%、准确度95.83%;诊断脑出血合并碘造影剂外渗的灵敏度94.44%、特异度89.47%、准确度93.94%。结论:急性脑梗死患者接受介入溶栓取栓术后尽早进行双能CT检查,可快速鉴别术后脑出血以及碘造影剂外渗情况,有利于临床及时处理,改善预后。Objective:To investigate the differential diagnostic value of dual-source CT scan for cerebral hemorrhage and extravasation of iodine contrast agent after interventional thrombolysis for acute cerebral infarction.Methods:select our hospital during May 2023 to August 2024,50 cases of acute cerebral infarction parallel interventional thrombolysis patients as the study object,patients are receiving dual-source CT double can scan within 2 hours after the treatment,then conventional brain CT examination,statistical analysis of dual-source CT double can scan for simple cerebral hemorrhage,iodine contrast and cerebral hemorrhage with iodine contrast onress diagnostic value of three different conditions(sensitivity,specificity and accuracy).Results:Conventional brain CT diagnosed 3 cases of simple cerebral hemorrhage,25 cases,18 cases,and 4 cases without cerebral hemorrhage and contrast agent extravasation within 24 to 72 hours after treatment.However,during 2 hours after dual source CT,the diagnosis of cerebral hemorrhage alone and without contrast was accurate;the sensitivity of iodine contrast alone was 92.00%,specificity 96.30%and accuracy was 95.83%;the sensitivity of diagnosis of cerebral hemorrhage with iodine contrast extravasation was 94.44%,specificity 89.47%and accuracy was 93.94%.Conclusion:Patients with acute cerebral infarction after interventional thrombectomy can quickly identify postoperative cerebral hemorrhage and iodine contrast agent extravasation,which is conducive to timely clinical treatment and improve the prognosis.
