摘　　要：目的:探讨多囊卵巢综合征(PCOS)患者经阴道彩色多普勒超声血流显像(CDFI)卵巢血流及血管形成特征及定量诊断价值。方法:纳入2022年1月至2024年1月收治的PCOS患者72例及同期健康体检产妇72例,均进行经阴道二维超声及CDFI检查,分析两组卵巢血流及血管形成特征,比较二维超声及CDFI定量参数差异,经ROC曲线分析诊断价值。结果:相比于对照组,观察组卵巢总体积、Vs及PI更大,RI更小,“项链征”、卵巢中央髓质回声增强率更高(P<0.05);经ROC曲线显示:二维超声及CDFI定量参数单一诊断PCOS发生均有一定价值,尤以联合诊断价值最高,AUC值为0.972,此时灵敏度、特异度依次为97.22%、97.22%。结论:经阴道CDFI检查可为PCOS患者提供卵巢血流及血管形成特征信息,在结合二维超声参数基础上,PCOS的诊断更为及时且准确。Objective:To investigate the characteristics of ovarian blood flowand angiogenesis and its quantitative diagnostic value in patients with polycystic ovary syndrome(PCOS)with transvaginal color Doppler ultrasound flow imaging(CDFI).Methods:A total of 72 patients with PCOS admited to our hospital from January 2022 to January 2024 and 72 healthy women in the same period during physical examination were selected as subjects.Transvaginal two-dimensional ultrasound and CDFI examination were performed to analyze the characteristics of ovarian blood flow and angiogenesis between the two groups,and the differences in quantitative parameters of two-dimensional ultrasound and CDFI were compared.The diagnostic value of single and combined 2D ultrasound and CDFI was analyzed by ROC curve.Results:Compared with the control group,the total ovarian volume,Vs and PI of observation group were larger,RI was smaller,the"necklace sign"and the echo enhancement rate of central medullary ovary were higher(P<0.05).The ROC curve showed that both two-dimensional ultrasound and CDFI quantitative parameters had certain value in the single diagnosis of PCOS occurrence,especilly the combination diagnosis with the highest value,AUC value was 0.972,and the sensitivity and specificity were 97.22%and 97.22%.Conclusion:Transvaginal CDFI examination can provide characteristic information of ovarian blood flow and vascularization in PCOS patients.Based on two-dimensional ultrasound parameters,the diagnosis of PCOS is more timely and accurate.
