摘 要:目的:评估可吸收骨蜡用于兔胸骨损伤模型的安全性和有效性,及对骨愈合的影响,并与传统不可吸收骨蜡进行比较。方法:选择18只雄性新西兰兔作为实验动物,随机平均为两组,一组为试验组,另一组为对照组,行胸骨正中切开术后,进行可吸收骨蜡/传统不可吸收骨蜡植入试验。术中评价止血效果。到观察终点——术后2周、4周、8周分别处死对应组的新西兰兔并解剖取胸骨,进行影像学分析、力学分析和组织病理学评价。结果:试验组使用可吸收骨蜡的方法,对照组使用常规方式进行止血,两组效果无显著性差异;各观察终点,试验组动物胸骨间隙均小于对照组动物,试验组动物的手术部位力学强度更高,表明与传统骨蜡相比,可吸收骨蜡不会阻碍骨创面的愈合;组织病理学结果显示,术后2周试验组新骨生长情况更好,术后4周骨创面基本愈合。术后2周、4周、8周试验组局部炎症反应记分均小于对照组。结论:可吸收骨蜡的止血效果与传统不可吸收骨蜡无差异,但可吸收骨蜡不会阻碍骨创面的愈合,并且在植入部位有更小的炎症反应。Objective:To evaluate the safety and efficacy of absorbable bone wax in rabbit sternal injury model and its effect on bone healing,in comparison with traditional non-absorbable bone wax.Methods:18 male New Zealand rabbits were randomly divided into test group and control group.After median sternotomy,absorbable bone wax/traditional nonabsorbable bone wax implantation tests were performed.The hemostatic effect was evaluated during operation.At 2 weeks,4 weeks and 8 weeks after operation,the New Zealand rabbits in the corresponding groups were sacrificed and the sternum was collected for computed tomography,mechanical testing and histological analysis.Results:The hemostatic effect of experimental group(6/9,66.7%)was not significantly different from that of control group(5/9,55.6%).At each observation end point,the sternal space of test group was smaller than that of control group,indicating that absorbable bone wax did not inhibit bone healing compared with traditional bone wax.The mechanical strength of the test group was higher than the control group.The histological results showed that the growth of bone cell in the test group was better at 2 weeks after operation,and the bone wound healed basically at 4 weeks after operation.Conclusion:The hemostatic effect of absorbable bone wax was not significantly different from that of traditional non-absorbable bone wax,but absorbable bone wax did not hinder the healing of bone wounds.
