艾司氯胺酮麻醉在儿童龋病舒适化治疗中的效果

摘　　要：目的:探究艾司氯胺酮麻醉在儿童龋病舒适化治疗中的应用。方法:于2023年1月—2024年6月选取我院收治的78例龋病患儿,采用单双号随机化法分为观察组和对照组,各39例。对照组用丙泊酚加舒芬太尼治疗,观察组采用艾司氯胺酮治疗。比较两组围术期指标、PAED评分、FLACC评分、不良反应。结果:两组镇静时间比较,差异无统计意义(P>0.05),观察组术中发生血氧饱和度低于90%的患儿例数少于对照组,苏醒时间以及PACU停留时间均短于对照组,治疗开始10 min的Ramsay镇静评分高于对照组(P<0.05);观察组苏醒后15 min、45 min的PAED和FLACC评分低于对照组(P<0.05);观察组不良反应发生率均低于对照组(P<0.05)。结论:儿童龋齿应用艾司氯胺酮舒适化治疗可有效改善苏醒质量,缓解患儿术后躁动及不良情绪,减轻疼痛程度,降低其不良反应发生。Objective:To explore the application of esketamine anesthesia in the comfortable treatment of dental caries in children.Methods:From January 2023 to June 2024,78 children with dental caries admitted to our hospital were selected and divided into observation group and control group by single and double number randomization method,with 39 children in each group.The control group was treated with propofol and sufentanil,and the observation group was treated with esketamine.Perioperative indicators,PAED score,FLACC score and adverse reactions were compared between the two groups.Results:There was no significant difference in sedation time between the two groups(P>0.05).The number of patients with blood oxygen saturation less than 90%in the observation group was less than that in the control group,the recovery time and PACU stay time were shorter than those in the control group,and the Ramsay sedation score at 10 min after treatment was higher than that in the control group(P<0.05).The PAED and FLACC scores of the observation group at 15 min and 45 min after recovery were lower than those of the control group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion:The comfortable treatment of esketamine in children's dental caries can effectively improve the quality of awakening,relieve the postoperative agitation and bad mood of children,reduce the pain degree,and reduce the occurrence of adverse reactions.
