摘　　要：目的:分析鲍曼不动杆菌对常用抗菌药物的耐药检测结果。方法:收集2021年1月—2023年12月我院患者痰液、血液、尿液、咽拭子、胸腹等分泌物标本,分析患者鲍曼不动杆菌检出率、标本、年龄、科室及抗菌药物耐药情况。结果:2021年1月—2023年12月我院临床分离细菌达到4872株,其中鲍曼不动杆菌株为333株,鲍曼不动杆菌检出率为6.83%,其中2021年检出率为6.34%(94/1482),2022年检出率为6.42%(108/1681),2023年检出率为7.67%(131/1709);经过对333株鲍曼不动杆菌标本进行分析发现,其中痰液最高,占比为67.87%,其次为咽拭子和尿液,占比分别为26.13%、3.00%;81~90岁人群鲍曼不动杆菌占比最高,其次为71~80岁和0~10岁,可见老年人群和儿童群体更加容易发生鲍曼不动杆菌感染;从科室鲍曼不动杆菌感染分布来看,以ICU感染率最高,达到26.73%,其次为呼吸内科和儿科,感染率分别为24.62%、21.92%;通过对鲍曼不动杆菌进行抗菌药物分析,对替加环素、米诺环素耐药率最低。结论:高龄和儿童群体容易发生鲍曼不动杆菌感染,且这种致病菌具有一定耐药性,需要加强监测力度,合理用药,降低患者感染率。Objective:To analyze the drug resistance test results of Acinetobacter baumannii to commonly used antibiotics.Methods:Sputum,blood,urine,throat swab,chest and abdomen secretion samples were collected from patients in our hospital from January 2021 to December 2023.The detection rate,specimen,age,department and antibiotic resistance of Acinetobacter baumannii were analyzed.Results:From January 2021 to December 2023,the number of clinically isolated bacteria in our hospital reached 4872 strains,including 333 strains of Acinetobacter baumannii,and the detection rate of Acinetobacter baumannii was 6.83%,among which 6.34%(94/1482)in 2021 and 6.42%(108/1681)in 2022.The detection rate in 2023 was 7.67%(131/1709).The analysis of 333 strains of Acinetobacter baumannii showed that sputum was the highest,accounting for 67.87%,followed by throat swab and urine,accounting for 26.13%and 3.00%,respectively.The proportion of Acinetobacter baumannii in 81-90 years old was the highest,followed by 71-80 years old and 0-10 years old,indicating that the elderly and children were more susceptible to Acinetobacter baumannii infection.From the distribution of Acinetobacter baumannii infection,ICU infection rate was the highest,reaching 26.73%,followed by respiratory medicine and pediatrics,infection rate was 24.62%and 21.92%,espectively.Through the analysis of antimicrobial agents for Acinetobacter baumannii,the resistance rate of tigacycline and minocycline was the lowest.Conclusion:Acinetobacter baumannii infection is easy to occur in the aged and children,and this pathogen has a certain drug resistance,so it is necessary to strengthen surveillance,rational drug use,and reduce the infection rate of patients.
