摘　　要：目的:探讨窄谱中波紫外线(NB-UVB)辅助治疗寻常型银屑病患者的效果及其对免疫功能、外周血Th17/Treg失衡及相关细胞因子表达的影响。方法:选取我科2019年10月—2022年12月期间87例寻常型银屑病患者作为研究对象,对照组43例给予消银片治疗,观察组44例增加NB-UVB辅助治疗,对比两组临床疗效、免疫功能(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))、外周血Th17/Treg失衡(Th17细胞、Treg细胞、Th17/Treg比值)、相关细胞因子表达(血清IL-17、IL-22、IL-23、TGF-β1水平)、不良反应。结果:观察组总有效率为90.91%,高于对照组的72.09%(P<0.05);观察组治疗后CD4^(+)、CD4^(+)/CD8^(+)、Treg细胞、TGF-β1高于对照组,CD8^(+)、Th17细胞、Th17/Treg、IL-17、IL-22、IL-23低于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:NB-UVB辅助治疗寻常型银屑病的疗效良好,对于改善患者炎症水平、提高免疫功能具有积极作用,且安全性较高。Objective:To investigate the effect of narrow-band ultraviolet B(NB-UVB)in the adjuvant treatment of patients with psoriasis vulgaris and its effect on immune function,Th17/Treg imbalance and related cytokine expression in peripheral blood.Methods:A total of 87 patients with psoriasis vulgaris in our department from October 2019 to December 2022 were selected as the research objects.43 patients in the control group were treated with Xiaoyin Tablets,and 44 patients in the observation group were treated with NB-UVB adjuvant therapy.The clinical efficacy,immune function(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)),peripheral blood Th17/Treg imbalance(Th17 cells,Treg cells,Th17/Treg ratio),related cytokine expression(serum IL-17,IL-22,IL-23,TGF-β1levels),and adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 90.91%,which was higher than 72.09%of the control group(P<0.05).After treatment,CD4^(+),CD4^(+)/CD8^(+),Treg cells and TGF-β1 in the observation group were higher than those in the control group,while CD8^(+),Th17 cells,Th17/Treg,IL-17,IL-22 and IL-23 were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:NB-UVB is effective in the adjuvant treatment of psoriasis vulgaris.It has a positive effect on improving the level of inflammation and immune function of patients,and has high safety.
