共同决策护理模式在丙种球蛋白治疗成人暴发性心肌炎期间的效果

摘　　要：目的:探究共同决策护理模式在丙种球蛋白治疗成人暴发性心肌炎中应用价值。方法:选取2022年1月—2023年12月我院接受丙种球蛋白治疗的90例成人暴发性心肌炎患者,依据随机数字表法分为研究组(45例)、参照组(45例)。参照组应用常规护理,研究组应用共同决策护理模式。比较两组患者决策困境[决策困境量表(DCS)]、焦虑和抑郁情绪[抑郁自评量表(SDS)、焦虑自评量表(SAS)]、中文版ICU意识模糊评估单(CAM-ICU)评分、ICU谵妄、治疗时间。结果:干预后,研究组DCS、SAS、SDS、CAM-ICU评分低于参照组,ICU谵妄发生率均低于参照组,差异有统计学意义(P<0.05);研究组ICU住院时间与总住院时间短于参照组,差异有统计学意义(P<0.05)。结论:成人暴发性心肌炎患者在丙种球蛋白治疗中配合实施共同决策护理模式,有助于缓解决策困境,消除负性情绪,预防ICU谵妄,缩短ICU住院时间,护理效果显著。Objective:To explore the application value of joint decision-making nursing model in adult patients with fulminant myocarditis during the gamma globulin treatment period.Methods:A total of 90 adult patients with fulminant myocarditis who received gamma globulin treatment in our hospital from January 2022 to December 2023 were selected and divided into study group(45 patients)and reference group(45 patients)according to random number table method.The reference group was given routine nursing,and the study group was given joint decision-making nursing mode.The decision-making dilemma[Decision Dilemma Scale(DCS)],anxiety and depression[Self-rating Depression Scale(SDS),Self-rating Anxiety Scale(SAS)],Chinese version of ICU Confusion Assessment Unit(CAM-ICU)score,ICU delirium and treatment time were compared between the two groups.Results:After intervention,the scores of DCS,SAS,SDS and CAM-ICU in the study group were lower than those in the reference group,and the incidence of ICU delirium was lower than that in the reference group,the differences were statistically significant(P<0.05).The ICU hospitalization time and total hospitalization time of the study group were shorter than those of the reference group,and the differences were statistically significant(P<0.05).Conclusion:For adult patients with fulminant myocarditis during the gamma globulin treatment period,joint decision-making nursing is helpful to alleviate the decision-making dilemma,eliminate negative emotions,prevent ICU delirium,shorten the length of ICU stay,and the nursing effect is significant.
