吴门医派儿科治疗慢性咳嗽的用药经验

摘　　要：儿童慢性咳嗽病因复杂,其中以咳嗽变异性哮喘和上气道咳嗽综合征最为常见。在吴门医派温病理论及络病理论指导下,吴门医派儿科认为“湿”“热”是江南地区儿童慢性咳嗽的常见病理因素,外湿与内湿相合,结于肺络,或湿热交争,上犯于肺,致使咳嗽迁延。提出“风、痰、瘀”是儿童慢性咳嗽难治的关键因素,“久病入络”,咳嗽日久,“风、痰、瘀”夹杂为患,阻塞肺络。在临证时采用清热化湿的治法,运用三焦辨证宣上、畅中、泄下,使湿邪从三焦分消;注重运用风药理论,选用性平味淡、无碍胃气之品祛风通络、散邪胜湿;注重化痰通络的治法,经方化裁清肺化痰,联合虫药通络镇咳;注重活血化瘀的治法,使用血肉有情之品破血行血、化痰散结、搜经剔络。附案例1则,以资验证。The etiology of pediatric chronic cough is complex,with cough variant asthma and upper airway cough syndrome being the most common.Guided by the theories of warm diseases and collateral diseases in the WU Medical School,the pediatric department of the WU Medical School believed that“dampness”and“heat”are common pathological factors in pediatric chronic cough in regions south of the Yangtze River.External dampness combines with internal dampness,accumulates in the lung collaterals,or interacts with heat,invading the lungs and leading to persistent cough.It is proposed that“wind,phlegm,and blood stasis”are key factors in the refractoriness of pediatric chronic cough.Prolonged cough leads to the involvement of the collaterals,with“wind,phlegm,and blood stasis”intermingling and obstructing the lung collaterals.In clinical practice,the treatment approach of clearing heat and resolving dampness is adopted,using the triple energizer syndrome differentiation to disperse and drain dampness from the upper,middle,and lower energizers.Attention is paid to the application of wind-dispelling herbs,selecting herbs with mild properties and flavors that do not impair stomach qi to dispel wind,unblock collaterals,disperse evil,and overcome dampness.Emphasis is placed on the treatment approach of resolving phlegm and unblocking collaterals,modifying classical prescriptions to clear the lungs and resolve phlegm,and combining with insect-derived medicines to unblock collaterals and relieve cough.Attention is also given to the treatment approach of promoting blood circulation to remove blood stasis,using substances derived from animals or humans to break up blood stagnation,resolve phlegm,disperse nodules,and search through meridians and collaterals.A typical medical case is attached for verification.
