B超引导下动静脉内瘘球囊扩张成形术治疗自体动静脉内瘘狭窄的效果观察

摘　　要：目的研究B超引导下动静脉内瘘(AVF)球囊扩张成形术治疗自体AVF狭窄的临床效果。方法队列研究。抽取2021年1月至2024年1月郑州人民医院收治的自体AVF狭窄患者82例, 按随机数字表法分为对照组和观察组, 每组41例。对照组采用内瘘切除重建术, 观察组采用B超引导下AVF球囊扩张成形术。比较两组术后3个月的内瘘通畅率、术后3 d的超声参数及并发症发生率。结果术后3个月, 观察组内瘘通畅率(92.68%, 38/41)高于对照组(73.17%, 30/41), P<0.05。术后3 d, 观察组透析血流量、内瘘自然流量、血管狭窄处内径均大于对照组(P均<0.05)。观察组并发症发生率(4.88%, 2/41)低于对照组(19.51%, 8/41), P<0.05。结论自体AVF狭窄患者采用B超引导下AVF球囊扩张术可取得良好效果, 能改善血管狭窄, 促进内瘘通畅, 且安全性较高。Objective To study the value of B-ultrasound-guided arteriovenous fistula(AVF)balloon angioplasty under in the treatment of autologous AVF stenosis.MethodsA total of 82 patients with autologous AVF stenosis treated in Zhengzhou People’s Hospital from January 2021 to January 2024 were selected for the cohort study.They were divided into the control group and the observation group according to the random number table method,with 41 cases in each group.The control group was treated by internal fistula resection and reconstruction,and the observation group was treated by B-ultrasound-guided AVF balloon angioplasty.The internal fistula patency rate 3 months after surgery,ultrasound parameters 3 d after surgery and the incidence of complications were compared between the two groups.ResultsThree months after surgery,the internal fistula patency rate of the observation group was 92.68%(38/41),which was higher than the 73.17%(30/41)of the control group(P<0.05).Three days after surgery the dialytic blood flow,natural flow of the internal fistula,and the inner diameter of the vascular stenosis in the observation group were larger than those in the control group(all P<0.05).The incidence of complications in the observation group was 4.88%(2/41),which was lower than the 19.51%(8/41)in the control group(P<0.05).ConclusionsThe use of B-ultrasound-guided AVF balloon angioplasty in patients with autologous AVF stenosis can achieve good outcomes,that is,it can improve vascular stenosis,and promote the patency of the internal fistula,with higher safety.
