活血通脉胶囊联合低分子肝素钙对老年膝关节骨性关节炎术后患者膝关节功能、凝血功能及并发症的影响

摘　　要：目的:探讨活血通脉胶囊联合低分子肝素钙对老年膝关节骨性关节炎(KOA)术后患者膝关节功能、凝血功能及并发症的影响。方法:选取2021年7月~2023年9月某院行全膝关节置换术(TKA)的120例老年KOA患者作为研究对象,根据随机数字表法分为对照组和观察组,每组60例。两组均进行常规治疗,对照组在常规治疗基础上给予低分子肝素钙治疗,观察组在对照组基础上加用活血通脉胶囊治疗。两组均治疗2周,并进行3个月随访。比较两组患者膝关节功能[美国特种外科医院(HSS)评分]、凝血功能(凝血酶原时间、部分凝血活酶时间、D-二聚体)、膝关节活动度及并发症发生情况。结果:治疗1个月和治疗3个月后,两组患者HSS评分均高于治疗前,且治疗3个月后HSS评分均高于治疗1个月后(P<0.05);观察组患者HSS评分均高于对照组(P<0.05)。治疗1周后,观察组患者凝血酶原时间长于对照组,D-二聚体低于对照组(P<0.05)。治疗2周后,两组患者最大伸直角度小于治疗前,且观察组小于对照组(P<0.05);最大屈曲角度均大于治疗前,且观察组大于对照组(P<0.05)。观察组并发症总发生率(3.33%)低于对照组(15.00%,P<0.05)。结论:活血通脉胶囊联合低分子肝素钙能显著改善行TKA的老年KOA患者膝关节功能和凝血功能,提升膝关节活动度,并降低并发症的发生率。Objective:To investigate the effect of Huoxue Tongmai capsules combined with low-molecular-weight heparin calcium on knee joint function,coagulation function and complications in postoperative patients with senile knee osteoarthritis(KOA).Methods:A total of 120 patients with senile KOA who underwent total knee arthroplasty(TKA)in a hospital from July 2021 to September 2023 were selected and assigned to the control group and observation group by random number table,with 60 patients in each group.Both groups were given conventional treatment.The control group was treated with low-molecular-weight heparin calcium in addition to the conventional treatment,while the observation group was treated with Huoxue Tongmai capsules in addition to the treatment given in the control group.Both groups were treated for 2 weeks and followed up for 3 months.The knee joint function[American Hospital for Special Surgery(HSS)score],coagulation function(prothrombin time,partial thromboplastin time,D-dimer),knee joint range of motion and complications were compared between the two groups.Results:At 1 month and 3 months after treatment,the HSS scores in the two groups were higher than those before treatment,and the HSS scores at 3 months after treatment were higher than those at 1 month after treatment(P<0.05).The HSS scores in the observation group were higher than those in the control group(P<0.05).After 1 week of treatment,the prothrombin time in the observation group was longer than that in the control group,and the D-dimer level was lower than that in the control group(P<0.05).After 2 weeks of treatment,the maximum extension angle in the two groups was smaller than that before treatment,with the observation group showing a smaller angle than the control group(P<0.05),and the maximum flexion angle in the two groups was greater than that before treatment,with the observation group showing a greater angle than the control group(P<0.05).The total incidence of complications in the observation group(3.33%)was lower than that in the contro
