摘　　要：目的:探讨免疫球蛋白A(IgA)肾病肾小管间质病变程度的影响因素。方法:选取2013年7月-2014年7月于广西医科大学第二附属医院就诊的221例IgA肾病患者进行研究,均进行病理检查,根据肾小管间质病变程度分为轻度组(n=164)和中重度组(n=57)。统计两组患者的一般资料,采用生化分析仪检测24 h尿蛋白定量、血肌酐、尿素氮、血清IgA水平,并检测TCRCα-560C/T多态性基因型,分析IgA肾病肾小管间质病变程度的影响因素。结果:24 h尿蛋白定量、肌酐、尿素氮是IgA肾病肾小管间质病变程度的独立影响因素。结论:临床针对IgA肾病肾小管间质病变患者,应加强24 h尿蛋白定量、肌酐、尿素氮指标监测,早期采取有效的干预措施,以延缓肾小管间质病变进展,改善患者预后。Objective:To explore the influencing factors for the degree of tubulointerstitial lesions in immunoglobulin A(IgA)nephropathy.Methods:A total of 221 patients with IgA nephropathy who visited The Second Affiliated Hospital of Guangxi Medical University from July 2013 to July 2014 were selected for the study.All patients underwent pathological examination.They were divided into mild group(n=164)and moderate-severe group(n=57)according to the degree of renal tubulointerstitial lesions.General data of patients in two groups were counted.The 24-h urinary protein quantification,blood creatinine,urea nitrogen and serum IgA levels were detected using biochemistry analyzer.TCRCα-560C/T polymorphism genotype was detected.The influencing factors for the degree of tubulointerstitial lesions in IgA nephropathy were analyzed.Results:The 24-h urinary protein quantification,creatinine,and urea nitrogen were independent influencing factors for the degree of tubulointerstitial lesions in IgA nephropathy.Conclusion:In clinical practice,for patients with tubulointerstitial lesions in IgA nephropathy,it is necessary to enhance the monitoring of 24-h urine protein quantification,creatinine and urea nitrogen,and take effective interventional measures at early stage to delay the progression of tubulointerstitial lesions and improve the prognosis of patients.
