复合保温法预防腹腔镜结直肠癌手术患者低体温的临床效果

摘　　要：目的:探讨复合保温法预防腹腔镜结直肠癌手术患者低体温的临床效果。方法:将2022年1-7月于广州医科大学附属番禺中心医院行腹腔镜结直肠癌手术的患者94例随机分为两组,每组47例。对照组应用常规保温法进行体温管理,试验组在对照组基础上应用复合保温法进行体温管理。比较两组干预效果。结果:入手术室时、麻醉诱导前、麻醉起效、入麻醉后恢复室时,两组体温比较,差异无统计学意义(P>0.05);麻醉后60 min、出麻醉后恢复室时体温,试验组体温高于对照组(P<0.05)。相较于对照组,试验组气管导管拔管时间更早,麻醉后恢复室停留时间更短(P<0.05)。试验组术后寒战发生率低于对照组,差异有统计学意义(P=0.026)。相较于对照组,试验组术后首次排气时间、术后首次排便时间、术后首次进食时间、术后首次下床活动时间更早,住院时间更短(P<0.05)。结论:复合保温法能够预防腹腔镜结直肠癌手术患者低体温、术后寒战,缩短气管导管拔管时间、麻醉复苏时间,促进患者术后恢复。Objective:To explore the clinical effect of composite thermal insulation method in preventing hypothermia in patients undergoing laparoscopic colorectal cancer surgery.Methods:A total of 94 patients who underwent laparoscopic colorectal cancer surgery in the Affiliated Panyu Central Hospital of Guangzhou Medical University from January 2022 to July 2022 were randomly divided into two groups,with 47 cases in each group.The control group used conventional thermal insulation method for body temperature management.The experimental group used composite thermal insulation method for body temperature management.The intervention effects between two groups were compared.Results:There was no statistical difference in body temperature between the two groups at the time of entering the operating room,before anesthesia induction,when anesthesia took effect,and at the time of entering post-anesthesia recovery room(P>0.05).The body temperature in the experimental group was higher than that in the control group at 60 min after anesthesia and at the time of leaving post-anesthesia recovery room(P<0.05).Compared with the control group,the experimental group had earlier endotracheal tube extubation time and shorter stay in the post-anesthesia recovery room(P<0.05).The incidence of postoperative chills in the experimental group was lower than that in the control group,and the difference was statistically significant(P=0.026).Compared with the control group,the experimental group had earlier time of first postoperative exhaust,first postoperative defecation,first postoperative eating,first postoperative ambulation,and shorter hospital stay(P<0.05).Conclusion:Composite thermal insulation method can prevent hypothermia and postoperative chills in patients undergoing laparoscopic colorectal cancer surgery,shorten endotracheal tube extubation time and anesthesia recovery time,and promote the postoperative recovery of patients.
