脉冲式正压冲管技术在持续膀胱冲洗患者中的应用研究

摘　　要：目的探讨在患者持续膀胱冲洗的过程中定时给予脉冲式正压冲管技术冲管对降低膀胱痉挛发生的临床效果。方法选取行TURP手术的96例患者,按照随机的方式分为对照组和实验组各48例。对照组术后常规持续膀胱冲洗,实验组术后在持续膀胱冲洗的基础上定时给予脉冲式正压冲管。比较2组患者术后膀胱痉挛的发生率及程度、膀胱冲洗时间、导尿管留置时间、术后平均住院时间、泌尿系感染的发生及焦虑程度。结果2组术后膀胱痉挛的发生率差异有统计学意义(P<0.05);2组患者术后膀胱冲洗时间、导尿管留置时间、术后平均住院时间及焦虑程度评分差异均有统计学意义(P<0.05)。2组患者术后泌尿系感染的发生差异无统计学意义(P>0.05)。结论脉冲式正压冲管技术联合持续膀胱冲洗相较于传统的单纯持续膀胱冲洗,可以降低膀胱痉挛的发生率,减少膀胱冲洗时间及尿管留置时间,减轻患者术后的焦虑情绪,缩短术后平均住院时间。Objective This study was designed to evaluate timed pulsed positive pressured manual bladder irrigation in patients with continuous bladder irrigation.Method A total of 96 patients,undergoing transurethral resection of the prostate(TURP),were divided into control group and experimental group with 48 patients in each group randomly.Continuous bladder irrigation was applied routinely in control group,while both modified manual and continuous bladder irrigation were used in experimental group.The incidence and degree of postoperative cystospasm were compared.Besides,duration of bladder irrigation,catheter indwelling time,postoperative hospital stay,incidence of urinary tract infection(UTI)and degree of anxiety were also compared between these two groups.Results The incidence of postoperative cystospasm was statistically significantly lower for experimental group(12.5%versus 37.5%,P<0.05).Besides,the duration of bladder irrigation,catheter indwelling time,postoperative hospital stay and anxiety degree were reduced in experimental group(all P<0.05).Furthermore,little changes were observed regarding incidence of postoperative UTI.Conclusion Compared to traditional simple continuous bladder irrigation,combination of pulsed positive pressured manual bladder irrigation could reduce incidence of postoperative cystospasm,shorten the duration of bladder irrigation,catheter indwelling time and postoperative hospital stay.In addition,it may relieve the postoperative anxiety of patients.
