超声心动图在儿童先天性心脏病筛查及随访中的应用价值

摘　　要：目的 探讨超声心动图在儿童先天性心脏病筛查及随访中的应用价值。方法 选取2018年1月至2022年12月由外院先天性心脏病筛查后转诊至苏州大学附属儿童医院进行超声心动图检查的3645例儿童作为研究对象,采用超声心动图进行确诊,并对其随访情况进行分析。结果 ①随访1年,3645例儿童中,确诊为先天性心脏病(CHD)的例数为433例(11.88%)。对确诊的433例CHD进行分型,其中单纯型210例,占比48.50%,复合型210例,占比48.50%,复杂型13例,占比3.00%。②初诊时间在2018年1月1日至2022年12月28日之间,室间隔缺损(VSD)患儿有142例,房间隔缺损(ASD)患儿有137例。142例VSD患儿中,其中室缺缺损大小<5mm共有93例,自愈率为39.78%(37/93),手术患者10.75%(10/93),随访未愈49.46%(46/93)。室缺缺损大小>5mm共有49例,自愈率为20.41%(10/49),手术患者34.69%(17/49),随访未愈44.89%(22/49)。137例ASD患儿中,房缺缺损相加总范围<5mm共有26例,其中筛孔14例,自愈率为42.86%(6/14),随访未愈57.14%(8/14);单孔12例,自愈率58.33%(7/12),随访未愈41.67%(5/14)。5~10mm共有98例,其中筛孔70例,自愈率为47.14%(33/70),手术患者1.43%(1/70),随访未愈51.43%(36/70);单孔28例,自愈率42.86%(12/28),手术患者28.57%(8/28),随访未愈28.57%(8/28)。>10mm共有13例,其中筛孔11例,自愈率45.45%(5/11),随访未愈54.55%(6/11);单孔2例,手术患者100.00%(2/2)。结论 超声心动图在儿童先天性心脏病筛查和随访中的应用价值显著,值得推广。Objective Purpose To explore the application value of echocardiography in the screening and follow-up of children with congenital heart disease.Methods From January 2018 to December 2022,3645 children who were referred to the Children's Hospital of Soochow University for echocardiography after congenital heart disease screening from other hospitals were selected as the study subjects.The echocardiography was used to confirm the diagnosis,and their follow-up was analyzed.Results①Among the 3645 children with 1-year follow-up,433(11.88%)were diagnosed with congenital heart disease(CHD).A total of 433 cases of CHD were classified,including 210 cases(48.50%)of simple type,210 cases(48.50%)of compound type,and 13 cases of complex type(3.00%).②The initial diagnosis period was from January 1,2018 to December 28,2022,and there were 142 children with ventricular septal defect(VSD)and 137 children with atrial septal defect(ASD).Among the 142 children with VSD,93 had a ventricular defect<5mm,and the spontaneous recovery rate was 39.78%(37/93),10.75%(10/93)underwent surgery,and 49.46%(46/93)cases were not healed at follow-up.There were 49 cases of ventricular defects>5mm,and the spontaneous healing rate was 20.41%(10/49),34.69%(17/49)underwent surgery,and 44.89%(22/49)failure to heal at follow-up.Among the 137 children with ASD,there were 26 cases with a total range of<5mm for atrial defects,including 14 cases with ethmoid holes,with a self-healing rate of 42.86%(6/14)and 57.14%(8/14)cases failure to heal at follow-up.There were 12 cases with a single well,and the selfhealing rate was 58.33%(7/12),and 41.67%(5/14)cases were not healed after follow-up.There were 98 cases of 5mm~10mm,including 70 cases with sieve holes,and the self-healing rate was 47.14%(33/70),1.43%(1/70)of surgical patients,and 51.43%(36/70)follow-up patients uncured.There were 28 single-port patients,with a self-healing rate of 42.86%(12/28),28.57%(8/28)of surgical patients,and 28.57%(8/28)follow-up patients uncured.There were 13 cases>10mm,including 11
