摘  要:目的 研究无创心功能仪联合超声心动图及血清BNP(BNP)在肿瘤治疗相关性心功能不全检测中的价值。方法 选取我院于2020年1月至2022年8月收治的80例纵隔原发性弥漫性大B细胞淋巴瘤患者,均开展无创心功能仪、超声心动图及血清BNP水平检测。对比放疗前与放疗后患者的每搏量(SV)、左室射血分数(LVEF)及血清BNP水平。按照是否并发心功能不全分为心功能不全组及心功能正常组,对比两组患者的SV、LVEF及血清BNP水平。以受试者工作特征(ROC)曲线分析SV、LVEF及血清BNP水平诊断肿瘤治疗相关性心功能不全的效能。结果 放疗后,SV、LVEF水平均低于放疗前,而BNP水平高于放疗前(P<0.05)。心功能不全组患者SV、LVEF水平均低于心功能正常组,而BNP水平高于心功能正常组(P<0.05)。经ROC曲线分析证实,SV、LVEF及血清BNP水平联合诊断肿瘤治疗相关性心功能不全的效能较之单一指标诊断更优。结论 无创心功能仪联合超声心动图及血清BNP对肿瘤治疗相关性心功能不全的检测价值较高,值得推广。Objective To study the value of non-invasive cardiac function instrument combined with echocardiography and serum B-type natriuretic peptide(BNP)in the detection of tumor treatment-related cardiac dysfunction.Methods A total of 80 patients with primary diffuse large B-cell lymphoma in the mediastinum admitted to our hospital from January 2020 to August 2022 were selected as the study subjects.Non-invasive cardiac function apparatus,echocardiography and serum BNP levels were detected in all subjects.Stroke output(SV&left ventricular ejection fraction(LVEF)and serum BNP levels were compared before and after radiotherapy.All subjects were divided into cardiac dysfunction group and normal cardiac function group according to whether there was cardiac dysfunction.The SV,LVEF and serum BNP levels of the two groups were compared.Receiver operating characteristic(ROC)curve was used to analyze the efficacy of SV,LVEF and serum BNP levels in the diagnosis of tumor-treatmentrelated cardiac dysfunction.Results The levels of SV and LVEF after radiotherapy were lower than those before radiotherapy,while the levels of BNP were higher than those before radiotherapy(P<0.05).The levels of SV and LVEF in the group with cardiac dysfunction were lower than those in the group with normal cardiac function,while the levels of BNP were higher than those in the group with normal cardiac function(P<0.05).ROC curve analysis confirmed that SV,LVEF and serum BNP levels combined in the diagnosis of tumor treatment-related cardiac dysfunction is more effective than single index diagnosis.Conclusion Non-invasive cardiac function instrument combined with echocardiography and serum BNP has a high value in the detection of tumor treatmentrelated cardiac dysfunction,which is worthy of promotion.
