摘　　要：目的 探究平板数字减影血管造影(DSA)低剂量模式在冠状动脉造影术中的效果和安全性。方法 选取2021年8月至2022年10月天津市胸科医院收治的行冠状动脉造影术患者125例,依据随机数表法分为对照组(62例)和研究组(63例),分别进行平板DSA常规剂量采集和低剂量采集模式。比较两组采集的图像质量、辐射剂量、并发症发生率,并对两组的冠状动脉造影图像进行分析。结果 研究组图像质量为1级的患者占比(84.13%)低于对照组(90.32%),研究组图像质量为2级、3级的患者占比分别为12.70%、3.17%,均高于对照组(8.06%、1.61%),但差异无统计学意义(P> 0.05)。两组患者并发症发生率比较,差异无统计学意义(P> 0.05);研究组患者自动控制曝光条件下累积剂量(CD)、剂量面积乘积(DAP)水平均低于对照组(P <0.05)。在透视状态下,研究组血管较为模糊,但两组均能满足临床诊断要求。结论 冠状动脉造影术中平板DSA低剂量模式与常规剂量模式的图像质量相当,同时在不增加患者术后并发症发生风险的前提下,能明显降低辐射剂量,对保障临床检查安全性具有重要价值,值得今后推广。Objective To explore the efficacy and safety of low dose mode digital subtraction angiography(DSA)in coronary angiography.Methods A total of 125 patients who underwent coronary angiography admitted to Tianjin Chest Hospital from August 2021 to October 2022 were selected,and were divided into two grouped according to random number table method,including the control group(62 cases)and the study group(63 cases).And the conventional dose collection and low-dose collection modes of flat-panel DSA were respectively performed.The image quality,radiation dosage and complication rate were compared between the two groups,and the coronary angiography images of the two groups were analyzed.Results The proportion of patients with.The proportion of patients with grade 2 image quality in the study group was 12.70%and 3.17%,respectively,higher than 8.06%and 1.61%in the control group,but the difference was not statistically significant(P>0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).The levels of cumulative dose(CD)and dosearea product(DAP)in the study group were lower than those in the control group(P<0.05).In the fluoroscopic state,the blood vessels of the study group were blurred,but both groups could meet the requirements of clinical diagnosis.Conclusion In coronary angiography,the image quality of low-dose mode of plate DSA is comparable to that of conventional dose mode.The radiation dosage can be significantly reduced without increasing the risk of postoperative complications,which has important value for ensuring the safety of clinical examination and is worthy of popularization in the future.
