早期渐进康复护理联合冰硝散外敷对尺桡骨骨干骨折手术患者患肢肿胀、疼痛和睡眠质量的影响

摘　　要：目的:探究早期渐进康复护理联合冰硝散外敷对尺桡骨骨干骨折手术患者患肢肿胀、疼痛和睡眠质量的影响。方法:选取2020年6月至2022年6月福建省泉州东南医院骨科收治的80例尺桡骨骨干骨折患者为研究对象,采用随机数字法分为对照组和联合组,每组40例。对照组患者给予早期渐进康复护理,联合组患者在对照组护理基础上联合冰硝散外敷干预。比较2组患者术后2周干预效果;术前、术后3d、术后2周患肢肿胀情况(肿胀周径差)、疼痛情况[疼痛视觉模拟评分法(VAS)]、睡眠质量[匹兹堡睡眠质量指数(PSQI)量表]差异。结果:术后2周,联合组优良率100%(40/40),对照组优良率90.00%(36/40),联合组优良率高于对照组(P<0.05)。术后3d和术后2周,2组肿胀周径差、疼痛VAS评分、PSQI评分均低于本组术前,睡眠效率高于本组术前(均P<0.05);术前、术后3d和术后2周,联合组肿胀周径差、疼痛VAS评分、PSQI评分均低于对照组,睡眠效率高于对照组(均P<0.05)。结论:对尺桡骨骨干骨折手术患者进行早期渐进康复护理联合冰硝散外敷干预效果更好,可加快患肢肿胀消退,减轻患者疼痛,提升睡眠质量,利于功能恢复、疾病转归。Objective:To explore the effect of early progressive rehabilitation nursing combined with external application of Bingxiao powder on limb swelling,pain and sleep quality of patients with ulnar and radial shaft fracture.Methods:From June 2020 to June 2022,a total of 80 patients with ulnar and radial shaft fractures were selected as research subjects,and they were divided into a control group and a combined group by random table dynamic random method,with 40 cases in each group.Patients in the control group were given early and progressive rehabilitation nursing,while patients in the combined group were intervened by external application of Bingxiao Powder on the basis of nursing in the control group.The intervention effects of two groups were compared at 2 weeks after operation.The swelling of the affected limb(circumference difference),pain level(visual analogue scale,VAS),and sleep quality(Pittsburgh Sleep Quality Scale,PSQI)were compared before operation,3 days after operation and 2 weeks after operation.Results:At 2 weeks after surgery,the excellent rate in the combined group was 100%(40/40),which was higher than that in the control group(90.00%,36/40)(P<0.05).At 3 days and 2 weeks after operation,the difference of swelling circumference,the scores of VAS and PSQI in the combined group were lower than those in the control group,and the sleep efficiency was higher than that in the control group(P<0.05).Compared to the control group,the combined group had significantly lower limb swelling,pain,and PSQI scores,as well as higher sleep efficiency,at all time points(preoperative,3 days after surgery,and 2 weeks after surgery)(all P<0.05).Conclusion:On the basis of early progressive rehabilitation nursing for patients with ulnar and radial shaft fractures,the intervention effect of Bingxiao Powder combined with external application is better,which can speed up the disappearance of limb swelling,relieve the pain of patients,improve the quality of sleep,facilitate functional recovery and disease prognosis,and is simple
