电针治疗耳鸣取穴规律及刺激参数的数据挖掘与思考

摘　　要：目的:研究电针治疗耳鸣的取穴及刺激参数规律。方法:检索中国知网、维普、万方、中国生物医学文献服务系统、Pubmed、Web of Science建库至2024年2月19日收录的电针治疗耳鸣的临床研究文献,建立耳鸣的电针取穴及刺激参数数据库,对数据进行描述性统计、关联规则分析与聚类分析。结果:对纳入的83篇文献中的83首处方进行分析,使用频次高的腧穴依次为听会、听宫、翳风、耳门、风池、率谷、完骨;腧穴归经以足少阳胆经、手少阳三焦经、手太阳小肠经、督脉为主;电针最常连接的穴对有听会-耳门、听会-听宫、听会-翳风;聚类分析得出5个聚类集群;听会-翳风-听宫-耳门、风池-供血、百会-晕听区、头窍阴-完骨、角孙-三阴交。关联规则中支持度最高的穴位组合是耳门-听会。最常使用的刺激参数为:连续波、30 Hz、电流强度以患者耐受为度、30 min/次、每周3次。结论:电针治疗耳鸣以局部取穴改善循环为主,重视电针波形、频率的选择,合理选取穴对。基于数据挖掘研究为临床电针治疗耳鸣取穴及参数选择提供参考,对研究过程中的发现提出思考与展望,为临床与科研更好地发展提供参考。Objective:To study the rules of acupoint selection and stimulation parameters of electroacupuncture treatment of tinnitus.Methods:The clinical literature of electroacupuncture treatment for tinnitus included in CNKI,VIP,Wanfang Sinomed,Pubmed,and Web of Science from the establishment of the database to February 19,2024 was searched.A database of electroacupuncture acupoint selection and stimulation parameters for tinnitus was established,and descriptive statistics,association rule analysis and cluster analysis were carried out on the data.Results:Eighty-three prescriptions in the 83 articles included were analyzed.The acupoints with high frequency of use were in order of Tinghui,Tinggong,Yifeng,Ermen,Fengchi,Shuaigu and Wangu.The acupionts mainly entered the gallbladder channel of foot-shaoyang,three-jiao channel of hand-shaoyang,small intestine channel of hand-taiyang and governor vessel.The most commonly connected acupoint pairs of electroacupuncture were Tinghui-Ermen,Tinghui-Tinggong and Tinghui-Yifeng.Cluster analysis yielded five clusters namely Tinghui-Yifeng-Tinggong-Ermen,Fengchi-Gongxue,Baihui-Yuntingqu,Touqiaoyin-Wangu and Jiaosun-Sanyinjiao.The acupoint combination with the highest support in the association rule was Ermen-Tinghui.The most commonly used stimulation parameters were continuous wave,30 Hz,current intensity as tolerated by the patient,30 min/times,and 3 times per week.Conclusion:Electroacupuncture treatment of tinnitus is mainly based on local acupoint selection to improve circulation and focused on the selection of electroacupuncture waveform and frequency.And the acupoint pairs should be selected reasonably.Based on the data mining research,it can provide reference for the selection of acupoints and parameters of electroacupuncture treatment of tinnitus in clinic.This paper puts forward the thinking and prospect of the findings in the research process,and provides reference for the better development of clinical and scientific research.
