滋潜通脉汤治疗阴虚血瘀型稳定型心绞痛临床疗效观察及其网络药理学研究

摘　　要：目的:观察滋潜通脉汤辅助西医常规疗法对阴虚血瘀型稳定型心绞痛患者(SAP)的疗效及运用网络药理学方法探究滋潜通脉汤潜在作用机制。方法:随机选取60例阴虚血瘀型SAP患者,随机分为对照组与治疗组,每组30名,对照组患者实施西药治疗方案(扩冠、抗血小板聚集、降脂),治疗组在对照组治疗基础上联合滋潜通脉汤,疗程8周。比较两组中医临床证候积分及临床疗效。联合网络药理学研究方法初步探究该方剂可能的作用机制。结果:治疗组在中医临床证候积分、临床疗效改善方面均优于对照组,差异具有统计学意义(P<0.05)。滋潜通脉汤可能通过Akt1等靶点作用于脂质与动脉粥样硬化、流体剪切应力与动脉粥样硬化等通路以及对氮化合物的反应、对氧水平的反应、细胞氧化还原酶活性等生物过程发挥治疗作用。结论:西医治疗方案联合滋潜通脉汤治疗阴虚血瘀型SAP能有效改善心绞痛中医临床证候积分及临床疗效,值得临床推广。Objective:To observe the efficacy of Ziqian Tongmai Decoction assisted by conventional western medicine in patients with stable angina pectoris(SAP)with Yin deficiency and blood stasis syndrome,and to explore the potential mechanism of Ziqian Tongmai Decoction by network pharmacology.Methods:60 patients with SAP with Yin-deficiency and blood stasis syndrome were randomly divided into control group and treatment group,with 30 patients in each group.Patients in the control group were treated with western medicines(coronary expansion,anti-platelet aggregation and lipid-lowering),and patients in the treatment group were treated with Ziqian Tongmai Decoction on the treatment basis of the control group.The course of treatment was 8 weeks.The clinical symptom scores and clinical efficacy of the two groups were compared.The potential mechanism of action of this formula was preliminarily explored by using network pharmacology.Results:The TCM clinical symptom scores and clinical efficacy improvement in the treatment group were better than those in the control group,and the differences were statistically significant(P<0.05).Ziqian Tongmai Decoction may exert its therapeutic effects through targets such as AKT1,acting on pathways related to lipids and atherosclerosis,fluid shear stress and atherosclerosis,as well as biological processes including responses to nitrogen compounds,responses to oxygen levels,and cellular redox enzyme activity.Conclusion:The western medicine treatment combined with Ziqian Tongmai Decoction can effectively improve the TCM clinical symptom scores and clinical effect of angina pectoris in SAP with Yin-deficiency and blood stasis syndrome,which is worthy of clinical promotion.
