田岳凤教授从“虚、寒、瘀”论治原发性痛经验案探析

摘　　要：原发性痛经是妇科的常见病、多发病,因其严重影响患者的生活和心理状态而广受关注。中医学称痛经为“经行腹痛”,其症状描述最早见于《金匮要略》,“带下经水不利,少腹满痛”。《景岳全书》言“经行腹痛,证有虚实”,中医学和现代医学对于原发性痛经病因病机的认识及治疗特色各有不同。历代医家认为痛经的病因繁多且复杂,或虚,或实,或虚实夹杂。田岳凤教授认为“虚、寒、瘀”是原发性痛经的主要致病因素,或因寒湿阻滞,或因气滞血瘀,或因气血虚弱使胞宫、冲任气血阻滞或失于濡养,从而导致“不通”或“不荣”而发生痛经。田岳凤教授通过温经汤方隔药饼灸配合针刺的方式来治疗痛经,发挥穴、药、灸三者协同作用,疗效显著。Primary dysmenorrhea is a common and frequently-occurring gynecological disease.It has been widely concerned because it seriously affects the life and psychological state of patients.Chinese medicine calls dysmenorrhea“abdominal pain during menstruation”.Its symptoms are first described in Synopsis of the Golden Chamber:“Morbid leukorrhea and poor flow of menstruation will cause pain in lateral lower abdomen.”Jingyue Quanshu says:“Abdominal pain during menstruation can be classified as syndromes of deficiency and excess.”Chinese medicine and modern medicine have different understanding of the etiology and pathogenesis of primary dysmenorrhea and treatment characteristics.Ancient Chinese doctors believe that the causes of dysmenorrhea are various and complicated.It may be because of the deficiency,excess,or the mixed deficiency and excess.Professor Tian Yuefeng believes that deficiency,cold,and blood stasis are the main pathogenic factors of primary dysmenorrhea.Dysmenorrhea may be caused by the block of cold-dampness,Qi stagnation and blood stasis,or the blockage of Qi and blood or loss of nourishment in uterus,chong channel and ren channel due to Qi and blood weakness,resulting in“obstruction”or“lack of nourishment”.Prof.Tian Yuefeng uses herbal cake-separated moxibustion with Wenjing Decoction combined with acupuncture to treat dysmenorrhea,which plays a synergistic effect of acupoints,medicine and moxibustion,and the curative effect is remarkable.
