摘　　要：糖尿病视网膜病变是糖尿病患者常见的微血管并发症之一,属中医学“消渴目病”范畴,随糖尿病病程进展其发病率不断增加,常致患者失明,给患者的生活造成极大不便。方朝晖教授从事糖尿病及其并发症相关临床研究工作三十余年,尤其对糖尿病血管病变的治疗独具见解。方教授认为本病是消渴发展至后期之变证,其基本病机离不开气阴两虚,兼以瘀血阻络。因此,在临床治疗中,方朝晖教授承袭新安医家固本培元的思想,重视“培补先后天之本,脾肾同治”的治疗思路,从脾肾论治用药的同时兼用活血药物,防止瘀血内生、阻滞络脉,标本同治,针对糖尿病视网膜早期病变患者具有较好的临床疗效。对方朝晖教授临证辨治糖尿病视网膜病变的思路及经验进行归纳总结,以供参考学习。Diabetic retinopathy is one of the common microvascular complications in diabetic patients,which belongs to the category of“xiaokemubing”in traditional Chinese medicine.Its incidence increases with the prolongation of the course of the disease,often leading to blindness and causing great inconvenience to the life of patients.Professor Fang Zhaohui has been engaged in clinical research on diabetes and its complications for more than 30 years,especially in the treatment of diabetic angiopathy.Prof.Fang believes that this disease is a syndrome of xiaoke transformed from the later stage of its development,and its basic pathogenesis is related to deficiency of both Qi and Yin,as well as blockage of collaterals by blood stasis.Therefore,in the clinical treatment,Prof.Fang Zhaohui inherits the idea of Xin′an doctors to firm the root and foster the source,pays attention to the treatment ideas of“cultivating the congenital root and the foundation of acquired constitution and treating spleen and kidney together”,and administers the drugs from spleen and kidney together with using blood-activating drugs to prevent the production of blood stasis from blocking the collaterals.This can achieve simultaneous treatment of principal and subordinate symptoms,and has a good clinical effect for patients with early diabetic retinopathy.This article summarizes Prof.Fang′s thoughts and experience in clinical syndrome differentiation and treatment of diabetic retinopathy for reference and study.
