超声引导下神经阻滞复合喉罩全麻在结肠癌手术中的应用效果分析

摘　　要：目的探讨超声引导下神经阻滞复合喉罩全麻在结肠癌手术中的应用效果及对患者血流动力学与术后不良事件的影响。方法选取2022年6月~2024年2月期间在伊犁哈萨克自治州奎屯医院行结肠癌手术治疗的180例患者进行研究,按照随机分配原则将其分为对照组(90例)和观察组(90例)。对照组实施喉罩全麻,观察组则采用超声引导下神经阻滞复合喉罩全麻。比较两组麻醉前(T_(0))、切皮时(T_(1))、术中10 min(T_(2))、手术结束时(T_(3))的平均动脉压(MAP)以及心率(HR),并比较两组苏醒质量、疼痛评分及术后不良事件。结果T_(0)时,两组MAP、HR比较差异不显著(P>0.05);T_(1)时,观察组MAP、HR均高于对照组,而在T_(2)、T_(3)时,观察组MAP、HR均低于对照组(P<0.05)。观察组自主呼吸恢复时间、睁眼时间以及麻醉恢复室停留时间均短于对照组(P<0.05)。术前,两组视觉模拟评分法(VAS)评分比较差异不显著(P>0.05);术后6 h、术后12 h,观察组VAS评分均低于对照组(P<0.05)。观察组术后不良事件总发生率(7.78%)低于对照组(17.78%)(P<0.05)。结论超声引导下神经阻滞复合喉罩全麻有助于维持结肠癌手术患者血流动力学稳定,提高苏醒质量,降低术后疼痛程度及不良事件发生风险。Objective To investigate the application of ultrasound-guided nerve block combined with laryngeal mask general anesthesia in colon cancer surgery and its effects on patients'hemodynamics and postoperative adverse events.Methods A total of 180 patients who underwent colon cancer surgery in Kuitun Hospital of YiLiHaSaKe Autonomous Prefecture from June 2022 to February 2024 were selected for study,and were divided into control group(90 cases)and observation group(90 cases)according to the principle of random distribution.The control group was treated with laryngeal mask general anesthesia,and the observation group was treated with ultrasound guided nerve block combined with laryngeal mask general anesthesia.The MAP and HR of the two groups were compared before anesthesia(T_(0)),at the time of incision(T_(1)),10 min during operation(T_(2)),and at the end of operation(T_(3)).The quality of recovery,pain score and postoperative adverse events were compared between the two groups.Results At T_(0),there was no significant difference in MAP and HR between the two groups(P<0.05).At T_(1),MAP and HR in the observation group were higher than those in the control group,while at T_(2) and T_(3),MAP and HR in the observation group were lower than those in the control group(P<0.05).The recovery time of spontaneous respiration,eye opening time and stay time of anesthesia recovery room in observation group were shorter than those in control group(P<0.05).Before operation,there was no significant difference in visual analogue scale(VAS)scores between the two groups(P<0.05).At 6 h and 12 h after operation,the VAS scores of the observation group were lower than those of the control group(P<0.05).The total incidence of postoperative adverse events in the observation group(7.78%)was lower than that in the control group(17.78%)(P<0.05).Conclusion Ultrasound-guided nerve block combined with laryngeal mask general anesthesia is helpful to maintain hemodynamic stability in patients undergoing colon cancer surgery,improve the quality of rec
