摘　　要：目的探讨5-氟尿嘧啶联合A型肉毒毒素对增生性瘢痕患者的疗效及对瘢痕血流灌注量、血清学指标的影响。方法选取2022年3月至2024年4月我院收治的86例增生性瘢痕患者为研究对象,随机分为单一组、联合组,各43例。单一组给予A型肉毒毒素治疗,联合组给予A型肉毒毒素+5-氟尿嘧啶治疗,两组均治疗3个月。对比两组治疗后疗效,治疗前后视觉模拟评分(VAS)、血流灌注量、温哥华瘢痕评分量表(VSS)、白细胞介素-2(IL-2)、血管内皮生长因子(VEGF)、肿瘤坏死因子-α(TNF-α)、表皮生长因子(EGF)、转化生长因子-β1(TGF-β1)水平,治疗期间不良反应发生率。结果联合组临床疗效高于单一组(P<0.05);治疗后联合组疼痛、瘙痒评分低于单一组(P<0.05);治疗后联合组瘢痕处厚度、血管分布、柔软度、色泽评分均低于单一组(P<0.05);治疗后联合组血流灌注量低于单一组(P<0.05);治疗后联合组IL-2、VEGF、TGF-β1、EGF、TNF-α水平低于单一组(P<0.05);两组不良反应发生率相比差异无统计学意义(P>0.05)。结论5-氟尿嘧啶联合A型肉毒毒素治疗增生性瘢痕患者安全有效,能降低血流灌注量,改善临床症状及瘢痕状态,抑制生长因子过度表达。Objective To investigate the efficacy of 5-fluorouracil combined with botulinum toxin type A in patients with hypertrophic scars and its effects on scar blood perfusion volume and serological indicators.Methods A total of 86 patients with hypertrophic scars who were admitted to our hospital from March 2022 to April 2024 were selected as the research subjects.The patients were randomly divided into a single group and a combined group,with 43 cases each.The single group was treated with botulinum toxin type A,while the combined group was treated with botulinum toxin type A and 5-fluorouracil.Both groups were treated for 3 months.The efficacy of the two groups after treatment,as well as the visual analog scale(VAS),blood perfusion volume,Vancouver scar scale(VSS),interleukin-2(IL-2),vascular endothelial growth factor(VEGF),tumor necrosis factor-α(TNF-α),epidermal growth factor(EGF),transforming growth factor-β1(TGF-β1)levels before and after treatment,and the incidence of adverse reactions during treatment zgylmr.org.cn 47 China Medical Cosmetology Vol.15 No.1 Jan.2025 China Medical Cosmetology Vol.15 No.1 Jan.2025 were compared.Results The clinical efficacy of the combined group was higher than that of the single group(P<0.05).After treatment,the pain and itching scores in the combined group were lower than those in the single group(P<0.05).After treatment,the thickness,vascular distribution,softness,and color scores of the scar in the combined group were lower than those in the single group(P<0.05).After treatment,the blood perfusion volume in the combined group was lower than that in the single group(P<0.05).After treatment,the levels of IL-2,VEGF,TGF-β1,EGF,and TNF-αin the combined group were lower than those in the single group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions The treatment of 5-fluorouracil combined with botulinum toxin type A for patients with hypertrophic scars is safe and effective,which can reduce blood perfus
