经典名方香苏散的古代文献分析与现代临床应用

摘　　要：目的系统梳理古今医籍文献中有关香苏散的条目和研究,了解香苏散的历史沿革以及现代临床应用,为其后续的临床研究提供理论基础。方法在古籍数据库《国医典藏》中以“香苏散”或“香苏饮”为关键词,在中国知网(CNKI)使用主题检索,时间为1982年—2022年,关键词为“香苏散”或“香苏饮”,根据纳排标准进行筛选,得到了香苏散的处方组成、药物炮制、功能主治、用药剂量以及现代临床应用。结果最终纳入39部古籍,涉及条目45条。香苏散首见于宋代《太平惠民和剂局方》,由香附(炒)、紫苏叶、陈皮、甘草(炙)4味药物组成,随着历史朝代更迭,沿用至今的处方组成上大多为香苏散加减方;用药剂量上,加减方与原方剂量比例不尽相同,药物炮制上,香附有6种炮制品,陈皮有2种(去白和不去白),甘草有2种(生甘草和炙甘草),功能主治略有变化,从治疗风寒感冒,由外到内,治疗胃肠疾病、水肿、气滞等;纳入44篇文献分析香苏散的现代临床应用,与《太平惠民和剂局方》的功能主治不同,香苏散在现代多用于治疗胃肠疾病。结论根据古今文献统计分析,香苏散及加减方处方组成灵活多变,用药剂量上不尽相同;现代注重原方中“治气滞”的功效,临床上大多应用于治疗胃肠疾病。Objective This paper aims to systematically review the entries and studies related to Xiangsu San in ancient and modern medical literature,learn about its historical evolution and modern clinical application,and provide a theoretical basis for its subsequent clinical research.Methods Selecting“Xiangsu San”or“Xiangsu Yin”as keywords,this paper conducts a thematic search on the China National Knowledge Infrastructure(CNKl)with a time set from 1982 to 2022,and then the paper screens the searched results according to the eligibility criteria,recording the prescription composition,drug preparation,functional indications,dosage,and modern clinical application.Results A total of 39 ancient books are ultimately included,involving 45 entries.Xiangsu San,first appearing in Prescriptions of Peaceful Benevolent Dispensary of the Song dynasty,is composed of four herbs of Cyperus(fried),Perilla leaf,orange peel,and licorice(roasted).With the changes in historical dynasties,most current prescriptions are modified based on the original Xiangsu San.As for the dosage,the dosage ratio of the modified formula to the original formula is not the same.As for the medicine processing,there are six processed products used in the preparation of Cyperus rotundus,two in the orange peel(whitening and non-whitening)and two in the licorice(roasted licorice and raw licorice),with slight differences in functional indications which include wind-cold-type common cold,gastrointestinal diseases,edema,gas stains and other diseases.In addition,different from the record in Prescriptions of Peaceful Benevolent Dispensary,Xiangsu San is clinically applied to treat gastrointestinal diseases as the major functional indication in modern times according to the analysis of the 44 pieces of literature.Conclusion According to the statistical analysis of ancient and modern literature,the composition and dosage of Xiangsu San and its modified formulas are flexible and variable.Modern medicines pay attention to the effect of treating gas stains in the origi
