摘  要:目的探究呼吸道合胞病毒(RSV)感染所致反复喘息患儿血清骨膜蛋白(Periostin)、嗜酸粒细胞来源神经毒素(EDN)与炎症细胞因子和再次喘息风险的关系,分析血清Periostin、EDN对再次喘息的预测价值。方法选择2020年12月-2022年12月秦皇岛市妇幼保健院收治的118例RSV感染所致反复喘息患儿,根据病情严重程度将其分为轻度组(n=35)、中度组(n=59)和重度组(n=24),比较3组患儿血清Periostin、EDN及炎症细胞因子[肿瘤坏死因子-α(TNF⁃α)、白细胞介素-8(IL⁃8)、IL⁃6]水平。Pearson检验分析血清Periostin、EDN与炎症细胞因子相关性。根据随访6个月是否发生再次喘息将患者分为非再次喘息组(n=99)与再次喘息组(n=16)。多因素Logistic回归模型分析再次喘息的危险因素。受试者工作特征(ROC)曲线分析血清Periostin、EDN对再次喘息的预测价值。结果重度组患儿血清Periostin、EDN、TNF⁃α、IL⁃8、IL⁃6水平显著高于中度组和轻度组,差异均有统计学意义(P均<0.05);中度组患儿血清Periostin、EDN、TNF⁃α、IL⁃8、IL⁃6水平显著高于轻度组,差异均有统计学意义(P均<0.05);3组患儿间血清Periostin、EDN、TNF⁃α、IL⁃8、IL⁃6水平比较差异均有统计学意义(F=56.277、15.190、35.265、12.950、21.017,P均<0.05)。Pearson检验显示,RSV感染所致反复喘息患儿血清Periostin与TNF⁃α、IL⁃8、IL⁃6成正相关(r=0.591、0.568、0.625,P均<0.001),血清EDN与TNF⁃α、IL⁃8、IL⁃6成正相关(r=0.602、0.582、0.595,P均<0.001)。再次喘息组血清Periostin、EDN、TNF⁃α、IL⁃8、IL⁃6、IgE水平、API阳性占比显著高于非再次喘息组,差异均有统计学意义(t/χ^(2)=-9.268、-6.067、-9.423、-2.106、-7.408、-35.170、4.626,P均<0.05)。血清Periostin、EDN、TNF⁃α、IL⁃8、IL⁃6、IgE水平升高及API阳性是RSV感染所致反复喘息患儿再次喘息的危险因素(OR=1.828、1.442、1.249、1.505、1.384、1.608、1.294,P均<Objective To explore the relationship between serum Periostin,eosinophil derived neurotoxins(EDN),inflammatory cytokines,and the risk of recurrent wheezing in children with recurrent wheezing caused by respiratory syncytial virus(RSV)infection,and analyze the predictive value of serum Periostin and EDN for recurrent wheezing.Methods A total of 118 children with recurrent wheezing caused by RSV infection admitted to the Qinhuangdao Maternal and Child Health Hospital from December 2020 to December 2022 were selected.The patients were divided into mild group(n=35),moderate group(n=59)and severe group(n=24).The serum levels of Periostin,EDN and inflammatory cytokines[tumor necrosis factor⁃α(TNF⁃α),interleukin⁃8(IL⁃8),and IL⁃6]of the three groups were compared.Pearson test was used to analyze the correlation between serum Periostin,EDN,and inflammatory cytokines.According to the 6⁃month follow⁃up,patients were divided into non⁃recurrent wheezing group(n=99)and recurrent wheezing group(n=16).Multivariate Logistic regression model was used to analyze the risk factors for recurrent wheezing.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum Periostin and EDN for recurrent wheezing.Results The serum levels of Periostin,EDN,TNF⁃α,IL⁃8 and IL⁃6 in severe group were significantly higher than those in moderate group and mild group(all P<0.05);the serum levels of Periostin,EDN,TNF⁃α,IL⁃8 and IL⁃6 in moderate group were significantly higher than those in mild group(all P<0.05);the serum levels of Periostin,EDN,TNF⁃α,IL⁃8 and IL⁃6 were significantly different among the three groups(F=56.277,15.190,35.265,12.950,21.017;all P<0.05).Pearson's test showed that serum Periostin was positively correlated with TNF⁃α,IL⁃8,and IL⁃6 in children with recurrent wheezing caused by RSV infection(r=0.591,0.568,0.625;all P<0.001),and serum EDN was positively correlated with TNF⁃α,IL⁃8,and IL⁃6(r=0.602,0.582,0.595;all P<0.001).The serum Periostin,EDN,T
