摘 要:目的探讨早期自然流产合并人巨细胞病毒(HCMV)感染患者血清γ-干扰素(IFN⁃γ)、可溶性白细胞介素2(sIL⁃2R)、淀粉样蛋白A(SAA)表达改变及其临床意义,旨在为临床完善相关诊疗机制提供参考。方法选取2018年1月-2022年1月黄冈市人民医院与黄冈市中医医院产科收治的早期自然流产合并HCMV感染患者100例为研究组,再选取同期在本院产科产检的健康孕妇42名为对照组;根据HCMV活动性将研究组分为活动性组(n=46)和非活动性组(n=54)。采用酶联免疫吸附试验法测定所有研究对象血清IFN⁃γ、sIL⁃2R、SAA表达水平,比较各组IFN⁃γ、sIL⁃2R、SAA表达水平。结果研究组血清IFN⁃γ、sIL⁃2R、SAA水平分别为(582.74±106.73)ng/L、(281.56±48.63)ng/L、(24.39±5.07)mg/L,均高于对照组(419.63±76.85)ng/L、(189.51±36.47)ng/L、(5.24±1.38)mg/L,差异均有统计学意义(t=8.968、11.025、24.062,P均<0.05);活动性组血清IFN⁃γ、sIL⁃2R、SAA水平分别为(624.19±95.43)ng/L、(305.61±42.49)ng/L、(29.38±6.18)mg/L,均高于非活动性组(547.43±110.86)ng/L、(261.24±45.68)ng/L、(20.14±4.96)mg/L,差异均有统计学意义(t=3.677、4.998、8.258,P均<0.05)。结论早期自然流产合并HCMV感染患者血清IFN⁃γ、sIL⁃2R、SAA呈高表达,且活动性感染表达水平显著升高,与HCMV感染病理过程密切相关,可作为产妇HCMV活动性感染的检测指标。Objective To investigate the changes of serum expression ofγ⁃interferon(IFN⁃γ),soluble interleukin⁃2(sIL⁃2R)and serum amyloid A(SAA)in women infected with human cytomegalovirus(HCMV)in early spontaneous abortion and its clinical significance,and provide reference for clinical improvement of relevant diagnosis and treatment mechanism.Methods From January 2018 to January 2022,100 cases of early spontaneous abortion complicated with HCMV infection were selected from the Obstetrics Department of Huanggang Municipal People's Hospital and Huanggang City Hospital of Traditional Chinese Medicine as the study group.Additionally,42 healthy pregnant women who underwent prenatal check⁃ups at our hospital during the same period were selected as the control group.Based on the activity of HCMV infection,the study group was further divided into active group(n=46)and non⁃active group(n=54).The levels of IFN⁃γ,sIL⁃2R,and SAA in the serum of all study participants were measured using enzyme⁃linked immunosorbent assay(ELISA),and the expression and significance of IFN⁃γ,sIL⁃2R,and SAA in HCMV⁃induced abortion were analyzed.Results The levels of serum IFN⁃γ,sIL⁃2R and SAA in study group were(582.74±106.73)ng/L,(281.56±48.63)ng/L,(24.39±5.07)mg/L,respectively,higher than those in control group[(419.63±76.85)ng/L,(189.51±36.47)ng/L,(5.24±1.38)mg/L](t=8.968,11.025,24.062;all P<0.05).The levels of serum IFN⁃γ,sIL⁃2R and SAA in the active group were(624.19±95.43)ng/L,(305.61±42.49)ng/L,(29.38±6.18)mg/L,respectively,higher than those in non⁃active group[(547.43±110.86)ng/L,(261.24±45.68)ng/L,(20.14±4.96)mg/L](t=3.677,4.998,8.258;all P<0.05).Conclusion The serum levels of IFN⁃γ,sIL⁃2R,and SAA were highly expressed in patients with early spontaneous abortion complicated with HCMV infection,and the expression levels in active infection were significantly increased,which were closely related to the pathological process of HCMV infection,and could be used as the detection index of maternal
