基于Kano模型的胃癌术后病人支持性照护需求及影响因素分析

摘　　要：目的:分析基于Kano模型的胃癌术后病人支持性照护需求及影响因素。方法:共纳入2022年1月—2023年11月医院收治的91例胃癌手术病人,采用癌症病人支持性照顾需求量表(SCNS-SF34)评估病人术后支持性照顾需求现状,分析胃癌术后病人支持性照护需求的影响因素;并运用Kano模型方法确定胃癌术后病人支持性照顾需求的属性。结果:91例胃癌手术病人经评估,SCNS-SF34量表得分为(98.85±8.73)分。不同文化程度、居住情况、恐惧疾病进展(FoP)资料的病人SCNS-SF34评分比较差异有统计学意义(P<0.05);不同其他资料的病人评分比较差异无统计学意义(P>0.05)。经线性回归分析结果显示,文化程度为初中及以下、独居、FoP严重均为胃癌术后病人支持性照护需求的影响因素(P<0.05)。经Kano模型分析,在SCNS-SF34量表中,魅力需求(A)有14项,必备需求(M)有9项,一元需求(O)有9项,无差异需求(I)有2项;34个条目中,病人对条目9、条目12、条目22、条目24、条目25、条目30满意度值更高,提示这些条目对病人满意度的影响较大。结论:胃癌术后病人支持性照护需求受病人文化水平、居住情况、FoP因素影响,且量表中条目9、条目12、条目22、条目24、条目25、条目30对病人满意度的影响较大。Objective:To analyze the supportive care needs and influencing factors of postoperative patients with gastric cancer based on Kano model.Methods:A total of 91 gastric cancer patients admitted to the hospital from January 2022 to November 2023 were included.The supportive care Need Scale for Cancer patients(SCNS-SF34)was used to evaluate the current situation of postoperative supportive care needs of patients with gastric cancer,and the influencing factors of supportive care need of patients with gastric cancer were analyzed.Kano model was used to determine the attributes of supportive care needs of postoperative patients with gastric cancer.Results:The score of SCNS-SF34 scale was(98.85±8.73)in 91 patients with gastric cancer after operation.Comparison of SCNS-SF34 scores among patients with different education level,living condition and FoP data(P<0.05);There was no significant difference in patient scores among different data(P>0.05).Through linear regression analysis,the results showed that the education level of junior high school or below,living alone,and the severity of FoP were all influencing factors for the supportive care needs of postoperative patients with gastric cancer(P<0.05).According to the analysis of Kano model,in the SCNS-SF34 scale,there are 14 items of charm need(A),9 items of essential need(M),9 items of univariate need(O),and 2 items of indifferent need(I).Among the 34 items,the satisfaction values of items 9,12,22,24,25 and 30 were higher,indicating that these items had greater impact on patient satisfaction.Conclusion:The supportive care needs of patients after gastric cancer surgery are affected by the cultural level,living conditions and FoP factors,and items 9,12,22,24,25 and 30 in the scale have greater influence on patient satisfaction.
