基于文献分析的胆木浸膏糖浆治疗呼吸道感染综合性评价  

Comprehensive Evaluation of Danmu Extract Syrup in the Treatment of Respiratory Tract Infections Based on the Literature Analysis

摘　　要：目的:通过文献分析对胆木浸膏糖浆治疗呼吸道感染进行综合性评价,为临床决策提供参考。方法:检索中国知网、万方数据库、Pubmed和CBM 4个数据库,调研胆木浸膏糖浆上市(2006年)至2023年5月的定性及定量证据,通过文献调研与meta分析相结合的方法从安全性、有效性、经济性、创新性、适宜性、可及性和中医药特色7个维度对胆木浸膏糖浆治疗呼吸道感染进行分析和评价。结果:安全性方面,meta分析结果显示与对照组相比,胆木组不良反应发生率更低[RR=0.42,95%CI:(0.29,0.59),P<0.05]。有效性方面,有5个临床指南推荐使用胆木浸膏糖浆治疗呼吸道感染;meta分析结果显示与对照组相比,胆木组的临床总有效率更高[RR=1.20,95%CI:(1.15,1.26),P<0.05],退热时间[MD=-29.17,95%CI:(-45.87,-12.47),P<0.05]和咽痛消失时间[MD=-1.16,95%CI:(-1.60,-0.71),P<0.05]更短。胆木浸膏糖浆日费用区间为20.5-41.0元,有药物经济学研究显示与小儿豉翘清热颗粒相比,胆木浸膏糖浆治疗急性上呼吸道感染更具有成本效果优势。结论:胆木浸膏糖浆安全性、有效性和经济性的证据较为充足,meta分析的评价结果较为确定,胆木浸膏糖浆的综合价值较高。Objective:To conduct a comprehensive evaluation of Danmu Extract Syrup for the treatment of respiratory tract infections through a literature analysis,so as to provide references for clinical decision-making.Methods:A search was performed in China National Knowledge Infrastructure(CNKI),Wanfang Database,PubMed,and CBM,investigating qualitative and quantitative evidence from the market introduction of Danmu Extract Syrup(2006)until May 2023.A combination of literature review and meta-analysis was employed to analyze and evaluate Danmu Extract Syrup across seven dimensions:safety,efficacy,economic viability,innovation,appropriateness,accessibility,and traditional Chinese medicine characteristics.Results:In terms of safety,meta-analysis results indicated that the incidence of adverse reactions in the Danmu group was significantly lower than that in the control group[RR=0.42,95%CI:(0.29,0.59),P<0.05].Regarding efficacy,five clinical guidelines recommend the use of Danmu Extract Syrup for respiratory tract infections.Meta-analysis results demonstrated that the clinical overall effective rate in the Danmu group was higher than in the control group[RR=1.20,95%CI:(1.15,1.26),P<0.05],with shorter time to fever reduction[MD=-29.17,95%CI:(-45.87,-12.47),P<0.05]and time for sore throat resolution[MD=-1.16,95%CI:(-1.60,-0.71),P<0.05].The daily cost of Danmu Extract Syrup ranged from 20.5 to 41.0 RMB,with pharmacoeconomic studies indicating a cost-effectiveness advantage over Xiaoer Chiqiao Qingre Granules for treating acute upper respiratory infections.Conclusion:Danmu Extract Syrup is demonstrated to be a safe and effective treatment for respiratory tract infections,with favorable economic viability and traditional medicine attributes.Further studies are recommended to strengthen these findings.
