摘  要:目的探讨脂蛋白相关的磷脂酶A2(Lp-PLA_(2))、糖化血红蛋白指数(HGI)与2型糖尿病视网膜病变严重程度的关系。方法选取2021年1月至2022年6月于湖南医药学院总医院住院的120例2型糖尿病患者作为研究对象,将其分为2型糖尿病无视网膜病变组(NDR组,n=45)、非增殖期糖尿病视网膜病变组(NPDR组,n=45)和增殖期视网膜病变组(PDR组,n=30)。同期随机抽取该院50例体检健康者作为对照组。收集并比较各组临床资料和Lp-PLA_(2)、HGI水平,并分析HGI及Lp-PLA_(2)水平与病情严重程度的相关性。结果NDR组、NPDR组、PDR组Lp-PLA_(2)、HGI均高于对照组(P<0.05)。高Lp-PLA组(H-Lp-PLA_(2)组)、高H-HGI组(H-HGI组)NPDR、PDR患者比例高于低HGI组(L-HGI组)和低L-Lp-PLA_(2)组(L-Lp-PLA_(2)组),差异有统计学意义(P<0.05)。多元Logistic回归分析显示,Lp-PLA_(2)、HGI是DR的危险因素(P<0.05),与2型糖尿病视网膜病变严重程度呈正相关(P<0.05)。结论HGI、Lp-PLA_(2)与2型糖尿病视网膜病变严重程度密切相关,可作为评估病情严重程度的新指标。Objective To investigate the correlation between lipoprotein-associated phospholipase A2(Lp-PLA_(2)),hemoglobin glycotion index(HGI)and the severity of type 2 diabetic retinopathy.Methods A total of 120 patients with type 2 diabetes mellitus who were hospitalized in the General Hospital of Hunan Medical University from January 2021 to June 2022 were selected and divided into type 2 diabetes without retinopathy group(NDR group,n=45),non-proliferative diabetic retinopathy group(NPDR group,n=45)and proliferative diabetic retinopathy group(PDR group,n=30).In the same period,50 healthy subjects were randomly selected as control group.The clinical data,Lp-PLA_(2) and HGI levels of each group were collected and compared,and the correlation between different HGI and Lp-PLA_(2) levels and the severity of the disease was analyzed.Results The levels of Lp-PLA_(2) and HGI in NDR,NPDR and PDR groups were higher than those in control group(P<0.05).The proportion of patients with NPDR and PDR in high-Lp-PLA group(H-Lp-PLA_(2) group)and high-HGI group(H-HGI group)were higher than those in low-HGI group(L-HGI group)and low-Lp-PLA_(2) group(L-Lp-PLA_(2) group),and the differences were statistically significant(P<0.05).Multiple Logistic regression analysis showed that Lp-PLA_(2) and HGI were risk factors for DR(P<0.05),and were positively correlated with the severity of type 2 diabetic retinopathy(P<0.05).Conclusion HGI and Lp-PLA_(2) are closely related to the severity of retinopathy in type 2 diabetes mellitus,and may be used as new indicators to predict the severity of the disease.
