摘 要:目的 探讨先兆子痫(PE)与甲状腺功能、肝肾功能指标的相关性,通过确定早期风险因素,旨在为预防PE的发展制定策略。方法 回顾性分析2019年1月至2022年3月本院诊断为孕中期PE但妊娠结局不同的孕妇。研究内容包括孕妇的甲状腺功能、肝功能和肾功能的检查结果,以及PE的发病统计。结果 正常孕妇与PE患者的甲状腺激素水平和肝肾功能差异有统计学意义(P<0.05)。孕早期孕妇的TSH、AST、Cr和BUN的水平低于孕晚期(P<0.05),FT3、FT4、ALT、ALB、STB和SDB的水平则高于孕晚期(P<0.05),这表明随着妊娠的进展,孕妇体内的TSH、AST、Cr和BUN水平逐渐升高,FT3、FT4、ALT、ALB、STB和SDB水平逐渐降低。此外,高龄孕妇更容易产生甲状腺功能减退和肝肾功能异常。FT3和ALB显示出较好的预测PE的价值。结论 孕妇甲状腺功能三项、肝功能五项和肾功能两项水平与妊娠结局具有显著相关性,孕期监测这些指标的变化对于临床制定针对性干预策略具有重要意义。特别是FT3和ALB指标的异常,可能是PE的危险因素。Objective To explore the natural course of preeclampsia and its relationship with thyroid,liver,and kidney function,and to identify early risk factors for the development of preeclampsia in order to formulate preventive strategies.Methods A retrospective analysis was conducted on pregnant women diagnosed with early-onset preeclampsia but with different pregnancy outcomes at Zhuji Maternal and Child Health Hospital from January 2019 to March 2022.The study included the examination results of thyroid function,liver function,and kidney function,as well as the incidence of preeclampsia.Results Comparison of thyroid function and liver and kidney function in pregnant women with different pregnancy outcomes showed that there was a statistically significant difference in thyroid hormone levels and liver and kidney function between normal pregnant women and those with preeclampsia(P<0.05).The levels of TSH,AST,Cr,and BUN in early pregnancy were lower than in late pregnancy(P<0.05),while the levels of FT3,FT4,ALT,ALB,STB,and SDB were higher in early pregnancy(P<0.05),indicating that as pregnancy progresses,the levels of TSH,AST,Cr,and BUN in pregnant women gradually increase,while the levels of FT3,FT4,ALT,ALB,STB,and SDB gradually decrease.Additionally,older pregnant women were more prone to hypothyroidism and liver and kidney dysfunction.FT3 and ALB showed good predictive value for preeclampsia(PE).Conclusion The levels of thyroid function(three items),liver function(five items),and kidney function(two items)in pregnant women are significantly correlated with pregnancy outcomes.Monitoring the changes in these indicators during pregnancy is of great significance for guiding clinical interventions.In particular,abnormalities in FT3 and ALB would be high-risk factors for early preeclampsia.
