冷冻胚胎移植术后血清雌二醇水平对妊娠结局影响

摘　　要：目的 探讨冷冻胚胎移植术后血清雌二醇水平对妊娠结局的影响。方法 回顾性分析2021年1月至2022年6月222例助孕患者的临床资料。根据移植后雌二醇使用剂量将患者分为雌二醇4 mg组、雌二醇6 mg组、雌二醇8 mg组、雌二醇6 mg+雌二醇/雌二醇地屈孕酮片1 mg组,根据妊娠结局分为妊娠组和妊娠失败组,比较各组患者的基本情况及胚胎移植后4 d血清雌二醇水平、孕酮水平,并构建ROC曲线分析雌二醇水平对妊娠结局的预测价值。结果 各雌二醇使用量组中妊娠和妊娠失败患者BMI、不孕年限比较均差异无统计学意义(P>0.05),妊娠失败组年龄均较妊娠组高,差异有统计学意义(P<0.05)。雌二醇4 mg、6 mg、8 mg组中妊娠失败患者移植术后4 d血清雌二醇浓度均较妊娠组高,孕酮浓度均较妊娠组低,但两组比较差异无统计学意义(P>0.05),雌二醇6 mg+雌二醇/雌二醇地屈孕酮片1 mg组中妊娠患者移植术后4 d血清雌二醇浓度均较妊娠失败组高,孕酮浓度较妊娠失败组低,但两组比较差异无统计学意义(P>0.05)。各组胚胎移植术后4 d血清雌二醇水平预测妊娠结局的ROC曲线下的面积均接近0.5(P>0.05)。结论 冷冻胚胎移植术后血清雌二醇水平不建议作为预测妊娠结局指标。Objective To explore the effect of serum estradiol levels on pregnancy outcomes after frozen embryo transfer(FET)surgery.Methods Retrospectively analysed the clinical data of 222 assisted reproductive patients from January 2021 to June 2022.Patients were divided into four groups based on the dosage of estradiol used after transplantation:the estradiol 4 mg group,the estradiol 6 mg group,the estradiol 8 mg group,the estradiol 6 mg+estradiol/estradiol dynorgestrel tablets 1 mg group.According to pregnancy outcomes,patients were divided into the pregnancy group and the pregnancy failure group.The basic information of each group of patients and their serum estradiol and progesterone levels at 4 days after embryo transfer were compared,and the receiver operating characteristic(ROC)curves were constructed to analyze the predictive value of estradiol levels on pregnancy outcomes.Results There was no statistically significant difference(P>0.05)in body mass index(BMI)and infertility duration between pregnancy and pregnancy failure patients in each estradiol usage group.The age of the pregnancy failure group was higher than that of the pregnancy group,and the difference was statistically significant(P<0.05).In the estradiol 4 mg,6 mg,and 8 mg groups,the serum estradiol concentration was higher and progesterone concentration was lower in patients with failed pregnancy after 4 days of transplantation compared to the pregnancy group,but the difference between the two groups was not statistically significant(P>0.05).In the estradiol 6 mg+estradiol/estradiol digestrone tablets 1 mg group,the serum estradiol concentration was higher and progesterone concentration was lower in patients with failed pregnancy after 4 days of transplantation compared to the pregnancy group,but the difference between the two groups was not statistically significant(P>0.05).The area under the ROC curve for predicting pregnancy outcomes based on serum estradiol levels 4 days after embryo transfer in each group was close to 0.5(P>0.05).Conclusion The se
