心脏磁共振在新型冠状病毒感染相关心肌炎随访中的价值

摘  要:目的 探讨心脏磁共振(CMR)在新型冠状病毒感染相关心肌炎患者随访中的价值。方法 选取2023年1月至8月新型冠状病毒感染相关心肌炎患者20例作为观察组,以及同期收治的CMR表现正常新型冠状病毒感染患者20例作为对照组。比较两组心脏结构、功能分析、心肌应变与特征定量,并分析不同指标的诊断效能。结果 两组左室舒张末期容积(LVEDV)、右室收缩末期容积(RVESV)、右室收缩末期容积指数(RVESVi)、细胞外容积(ECV)、T_(2)值、右室射血分数(RVEF)及整体纵向应变(GLS)比较,差异有统计学意义(P<0.05)。受试者操作特征(ROC)曲线显示,T_(2)值的AUC值为0.79,敏感度为60%,特异度为95%,效能最佳。结论 新型冠状病毒感染相关心肌炎患者随访中发现,CMR提示有心室增大、功能下降及心肌特征异常,有助于心肌损伤后的疾病诊治。Objective To investigate the value of cardiac magnetic resonance(CMR)in the follow-up of patients with novel coronavirus infection associated myocarditis.Methods From January 2023 to August 2023,20 patients with novel coronavirus infection related myocarditis were selected as the study group,and 20 patients with normal CMR performance received at the same time were selected as the control group.Compared the analysis of cardiac structure,function,myocardial strain,and feature quantification between two groups,and analyze the diagnostic efficacy of different indicators.Results There were statistically significant differences(P<0.05)in left ventricular end diastolic volume(LVEDV),right ventricular end systolic volume(RVESV),right ventricular end systolic volume index(RVESVi),extracellular volume(ECV),T_(2) value,right ventricular ejection fraction(RVEF),and overall longitudinal strain(GLS)between the two groups.The receiver operating characteristic(ROC)curve showed that the AUC value of T_(2) value was 0.79,sensitivity was 60%,specificity was 95%,and the efficacy was the best.Conclusion In the follow-up of patients with novel coronavirus infection associated myocarditis,CMR indicates that there are ventricular enlargement,functional decline and abnormal myocardial characteristics,which is helpful for clinical disease management after myocardial injury.
