摘 要:目的 探讨磁共振非对称回波的最小二乘估算法迭代水脂分离(MR IDEAL-IQ)技术分析腰椎椎体骨髓脂肪分数(FF)在原发性骨质疏松症中的应用价值。方法 回顾性分析2021年8月至2023年3月59例行双能X线吸收法(DXA)骨密度测定和腰椎MRI检查患者的临床资料,测量椎体(L_(1~4))骨髓脂肪分数(FF)和骨密度(常规用T值)。根据T值分为骨质正常组、骨质减少组和骨质疏松组,在IDEALIQ序列的脂肪分数图像上测量椎体(L_(1~4))骨髓脂肪分数(FF)。比较各组间FF的差异,并分析FF与DXA骨密度测定值的相关性。结果 各组信号FF值分别为:骨质正常组(54.68%±12.32%),骨质减少组(58.95%±6.16%),骨质疏松组(67.85%±8.48%)。L_(1)椎体FF值在三组中均最低,L_(3)椎体FF值在骨质疏松组和骨量减少组中较高。骨质正常组FF值与骨质减少组比较,差异无统计学意义(P>0.05),骨质正常组FF值与骨质疏松组间差异有统计学意义(P<0.05)。骨质减少组FF值与骨质疏松组间差异有统计学意义(P<0.05)。腰椎FF与骨密度T值呈负相关(r=-0.427,P<0.05)。L_(1~4)椎体FF值在骨质正常组与骨质疏松组比较差异有统计学意义(P<0.05)。结论 采用MR IDEAL-IQ技术定量分析腰椎椎体骨髓脂肪含量的变化,可以为原发性骨质疏松症提供有效的诊断价值。Objective To investigate the value of MR(Iterative Decomposition of Echoes by Least-squares Estimation Quantification,IDEAL-IQ)fat quantification technique to analyze lumbar vertebral bone marrow fat fraction(FF)in primary osteoporosis.Methods Fifty-nine subjects who underwent dual-energy X-ray absorptiometry(DXA)bone densitometry and lumbar MRI at Xiaoshan Hospital of Wenzhou Medical University from August 2021 to March 2023 were retrospectively analyzed for measurement of vertebral body(L_(1~4))bone marrow fat fraction(FF)and bone mineral density(routinely T-value)was used.According to the T value,the subjects were divided into the normal bone mass group,the reduced bone mass group and the osteoporosis group.Measured the vertebral(L_(1~4))marrow fat fraction(FF)on fat fraction images of the IDEAL-IQ sequence.Compared the differences in FF among groups and analyzed the correlation between FF and dual-energy X-ray absorptiometry(DXA)bone density measurements.Results The FF values for each group were as follows:the normal bone mass group(54.68%±12.32%),the reduced bone mass group(58.95%±6.16%),and the osteoporosis group(67.85%±8.48%).The L_(1) vertebrae FF value was the lowest in all three groups,and the L_(3) vertebrae FF value was the highest in the osteoporotic and reduced bone groups.The P value between the normal bone mass group and the reduced bone mass group was 0.15,and the difference was not statistically significant(P>0.05).The difference between the normal bone mass group and the osteoporosis group was statistically significant(P<0.05).The difference between the reduced bone mass group and the osteoporosis group was statistically significant(P<0.05).The lumbar vertebral body FF was negatively correlated with the bone densitometry T value(r=-0.427,P<0.05).L_(1~4) vertebral body FF values were statistically significant(P<0.05)when compared between the normal bone group and the osteoporotic group.Conclusion Quantitative analysis of the changes in bone marrow fat content in lumbar vertebrae using magneti
