早产儿胎粪菌群多样性及富集性与早产儿发生宫外生长迟缓的关联分析

摘  要:目的 观察早产儿胎粪菌群的多样性及富集性与早产儿宫外生长迟缓的关系。方法 采用病例对照研究,纳入2022年6月至2023年5月在银川市妇幼保健院医院分娩、收住新生儿重症监护室且胎龄<33周的早产胎儿94例,收集临床资料及胎粪样本。依据早产儿纠正胎龄至33周或出院时体质量是否小于第10百分位分为宫外生长发育迟缓(EUGR)组和非EUGR组,采用16 S r DNA测序,经生物信息学分析比较2组胎粪样本肠道菌群的差异。结果 在α多样性指数方面,EUGR早产儿在Chao1指数、Shannon指数、Pielou_e指数均高于非EUGR组(P均<0.001)。在β多样性方面,EUGR组早产儿胎粪菌群在物种多样性上高于非EUGR组(R=0.122,P=0.002),在门水平(R=0.268,P=0.001)、纲水平(R=0.258,P=0.001)、目水平(R=0.115,P=0.002)、科水平(R=0.121,P=0.002)、属水平(R=0.119,P=0.001)、种水平(R=0.376,P=0.001)上,EUGR组均高于EUGR组(P均<0.01);基于物种OTUs的主坐标分析显示,EUGR组早产儿样本在物种多样性方面高于非EUGR组(P<0.01)。EUGR组早产儿胎粪肠道元基因组菌、死亡梭杆菌、不可培养的厚壁菌门、不可培养的拟杆菌、巴氏拟杆菌属目、不可培养的杆菌、无法分类的微生物丰度均高于非EUGR组(P均<0.05)。结论 EUGR早产儿肠道微生物群特征性明显。研究肠道微生物群在早产儿宫外生长迟缓中的作用,对于改善早产儿健康具有重要意义。Objective To investigate the relationship between Diversity and Enrichment of Meconium Microflora and Extrauterine Growth Retardation in premature infants.Methods A case control study was conducted to include clinical data and biological information of meconium samples from premature infants born in Yinchuan Maternal and Children Healthcare Hospital from June 2022 to May 2023,admitted to neonatal intensive care unit and had a gestational age of less than 33 weeks,total 94 cases.Premature infants were divided into the EUGR group and the non EUGR group based on whether the corrected gestational age reached 36 weeks or the weight at discharge was less than the 10th percentile.Using 16S rDNA sequencing,the differences in gut microbiota between two groups of meconium samples were compared through bioinformatics analysis.Results In terms ofαdiversity index,the Chao1 index,Shannon index,and Pielou_e index were significantly higher in EUGR premature infants than those of the non-EUGR group(P all<0.001).In terms ofβdiversity,the EUGR group of premature infant samples showed higher species diversity than the non EUGR group(R=0.122,P=0.002),and higher species abundance at the phylum level(R=0.268,P=0.001),class level(R=0.258,P=0.001),order level(R=0.115,P=0.002),family level(R=0.121,P=0.002),genus level(R=0.119,P=0.001),and species level(R=0.376,P=0.001)compared to the non-EUGR group(P<0.01).PCOA based on species OTUs showed that premature infant samples in the EUGR group have higher species diversity than those in the non-EUGR group(P all<0.01).Premature infants in the EUGR group had higher fecal gutmetagenome,Fusobaterium mortiferum,uncultured Firmicutes,uncultured Bacteroides,Bacteroidales amesiae,uncluttered bacterium,and unclassified microbial abundance than those in the non-EUGR group(P all<0.05).Conclusion The characteristics of gut microbiota in premature infants with EUGR are significant.Further research should be conducted on the role of gut microbiome in extrauterine growth retardation of premature infants.
