仙灵骨葆胶囊联合普伐他汀钠片在早中期SONFH患者术后治疗中的应用效果

摘　　要：目的 观察仙灵骨葆胶囊联合普伐他汀钠片在早中期激素性股骨头坏死(SONFH)患者术后治疗中的应用效果。方法 选取2021年3月—2022年12月广西医科大学第三附属医院收治的行髓芯减压植骨术的早中期SONFH患者90例,采用随机数字表法分为仙灵骨葆组、普伐他汀组和联合用药组,各30例。术后3组患者均行常规治疗,在此基础上,仙灵骨葆组口服仙灵骨葆胶囊,普伐他汀组服用普伐他汀钠片,联合用药组采用仙灵骨葆胶囊联合普伐他汀钠片,3组均用药3个月,随访(16.7±3.2)个月。比较3组用药前及末次随访时髋关节功能评分标准(Harris)评分、视觉模拟评分法(VAS)评分、股骨头坏死面积,股骨头存活率、并发症发生情况。结果 末次随访时,3组Harris评分高于用药前,VAS评分低于用药前,股骨头坏死面积小于用药前,联合用药组改善幅度大于仙灵骨葆组、普伐他汀组(P<0.05),但仙灵骨葆组与普伐他汀组Harris、VAS评分与股骨头坏死面积比较差异无统计学意义(P>0.05)。仙灵骨葆组、普伐他汀组、联合用药组股骨头存活率比较,差异无统计学意义(6.66%vs. 3.33%vs. 3.33%,χ^(2)=0.523,P=0.770)。患者术后均未出现切口感染、深静脉血栓形成等严重并发症。结论 早中期SONFH患者术后采用仙灵骨葆胶囊联合普伐他汀钠片治疗效果优于单独用药,可更有效改善髋关节功能,减轻疼痛,减小股骨头坏死面积,且预后较好。Objective To observe the application effect of Xianling Gubao capsules combined with pravastatin sodium tablets in the treatment of early and middle stage of SONFH.Methods From March 2021 to December 2022,90 cases of early and middle stage of SONFH who underwent core decompression and bone grafting were selected in the Third Affiliated Hosiptal of Guangxi Medical University,and they were divided into the Xianling Gubao group,pravastatin group and the combination group,30 cases in each group.Three groups of patients were given routine treatment after operation,on the basis of this,patients in the Xianling Gubao group were given Xianling Gubao capsules,patients in the pravastatin group were given pravastatin sodium tablets,and patients in the combination group were given Xianling Gubao capsules combined with pravastatin sodium tablets,all patients were medicated for 3 months,and followed-up for(16.7±3.2)months.Harris score,VAS score,and area of femoral head necrosis before medication and at the last follow-up visit,the survival rate of femoral head and incidence of complications were compared among the three groups.Results At the last follow-up visit,Harris score among the three groups was higher than that before medication,VAS score was lower than that before medication,area of femoral head necrosis was less than that before medication,and the indexes in the combination group were better than those in the Xianling Gubao group and the pravastatin group(P<0.05),and there was no significant difference in the Harris score,VAS score,area of femoral head necrosis between the Xianling Gubao group and the pravastatin group(P>0.05).There was no significant difference of survival rate of femoral head among the Xianling Gubao group,the pravastatin group and the combination group(6.66%vs.3.33%vs.3.33%,χ^(2)=0.523,P=0.770).There was no serious complications such as incision infection or deep vein thrombosis after operation.Conclusion The curative effect of Xianling Gubao capsules combined with pravastatin sodium tablets in e
