摘　　要：目的:研究阑尾黏液性肿瘤的超声声像图的特征,分析评估常规术前超声检查对阑尾黏液性肿瘤的诊断价值。方法:回顾性分析2010年1月~2024年6月泉州市第一医院收治的经病理诊断且术前于本院行超声检查的阑尾黏液性肿瘤患者17例,分析患者的超声声像图特征,总结阑尾黏液性肿瘤的超声特征。结果:(1)肿瘤位置相对固定,常位于右下腹。(2)阑尾黏液性肿瘤的超声声像表现可分三型:盲管状型(11.8%),以囊性为主的包块型(41.2%),以囊实性为主的包块型(47.1%)。(3)囊性成分内部透声欠佳,呈“洋葱皮样”和“胶冻样”改变;盲管状型管腔直径>15mm,包块型直径>20mm;囊内分隔多为不完全分隔;囊壁、分隔上及实性成分少见彩色血流信号;内部可见强回声伴彗尾征(囊壁钙化)。(4)肿瘤周边炎症反应轻微。结论:阑尾黏液性肿瘤在超声声像上具有一定特征性,超声在术前诊断中有一定提示作用。Objective:To explore the ultrasonographic features of appendiceal mucinous tumors and assess the diagnostic value of ultrasonography for these tumors.Methods:A retrospective analysis was conducted on 17 patients with appendiceal mucinous tumors who were pathologically diagnosed and underwent preoperative ultrasonography at the First Hospital of Quanzhou from January 2010 to June 2024.The ultrasonographic features of the patients were analyzed to summarize the ultrasonographic characteristics of appendiceal mucinous tumors.Results:①The tumor location was relatively fixed,often in the right lower abdomen.②The ultrasonographic manifestations of appendiceal mucinous tumors can be divided into three types:blind tube type(11.8%);Cystic dominant mass type(41.2%);And cystic-solid dominant mass type(47.1%).③The internal echo of the cystic components was poor,showing“onion skin-like”and“jelly-like”changes;The diameter of the blind tube type was greater than 15mm,and the mass type was greater than 20mm;The cystic septa were mostly incomplete;The cyst walls,septa,and solid components rarely show color blood flow signals;Strong echoes with comet tail signs(cyst wall calcification)can be seen inside.④The peripheral inflammatory response of the tumor was mild.Conclusion:Appendiceal mucinous tumors have certain characteristic features on ultrasonography,and ultrasonography plays a certain indicative role in preoperative diagnosis.
