摘  要:目的:探讨UF-1000i尿沉渣细胞检测仪红细胞多参数在血尿来源患者中的应用效果及价值。方法:选择2020年5月~2022年5月血尿来源患者69例为对象,采用UF-1000i尿沉渣细胞检测仪测定红细胞多参数,绘制ROC曲线,分析不同红细胞多参数在血尿来源患者中的诊断效能;对入组病例均完成相差显微镜观察红细胞形态学分类计数,分析其与UF-1000i尿沉渣细胞检测仪符合率和特异性。结果:肾小球性血尿患者RBCCMFs、RBC-P70Fsc低于非肾小球性血尿和混合性血尿;RBC-Fsc-DW高于非肾小球性血尿(P<0.05);低于混合性血尿(P<0.05);ROC曲线结果表明,UF-1000i尿沉渣细胞检测仪红细胞参数RBCCMFs、RBC-P70Fsc联合RBC-Fsc-DW在不同血尿来源患者中灵敏度及特异度高于单一参数(P<0.05);UF-1000i尿沉渣细胞检测鉴别肾小球性血尿和非肾小球性血尿灵敏度为79.17%(19/24)、特异度为92.00%(23/25)、阳性预测值90.48%(19/21)、阴性预测值为82.14%(23/28)。结论:UF-1000i尿沉渣细胞检测仪红细胞多参数在不同来源血尿患者中存在差异,不同指标联合检测具有较高的诊断效能,且与相差显微镜检测相比存在高度相关性,能为临床诊疗提供依据。Objective:To explore the application effect and value of UF-1000i urine sediment cell detector with multiple parameters of red blood cells in patients with hematuria.Methods:69 patients with hematuria from May 2020 to May 2022 were selected as the subjects.The UF-1000i urine sediment cell detector was used to measure the multiple parameters of red blood cells,and ROC curves were drawn to analyze the diagnostic efficacy of different red blood cell parameters in patients with hematuria;All enrolled cases were subjected to phase contrast microscopy to observe the morphological classification and counting of red blood cells,and their compliance and specificity with the UF-1000i urine sediment cell detector were analyzed.Results:RBCCMFs and RBC-P70Fsc in patients with glomerular hematuria were lower than those in non glomerular hematuria and mixed hematuria;RBC-Fsc-DW was higher than non glomerular hematuria(P<0.05);Lower than mixed hematuria(P<0.05);The ROC curve results showed that the red blood cell parameters RBCCMFs,RBC-P70Fsc combined with RBC-Fsc-DW of the UF-1000i urine sediment cell detector were more sensitive and specific than single parameters in patients with different sources of hematuria(P<0.05);The sensitivity of UF-1000i urine sediment cell detection in distinguishing glomerular hematuria from non glomerular hematuria was 79.17%(19/24),specificity was 92.00%(23/25),positive predictive value was 90.48%(19/21),and negative predictive value was 82.14%(23/28).Conclusion:The UF-1000i urine sediment cell detector has differences in red blood cell parameters among patients with hematuria from different sources.The combined detection of different indicators has high diagnostic efficacy and is highly correlated with phase contrast microscopy detection,which can provide a basis for clinical diagnosis and treatment.
